摘要
目的通过颈外静脉与肘部静脉穿刺置入中心静脉导管(PICC)血栓形成及感染的对比分析,探讨二者行中心静脉置管的优缺点,为PICC置管的最佳途径提供临床依据。方法将2013年1月-2014年1月在医院肿瘤科住院行PICC的604例患者按置管部位不同分为两组,试验组330例选择颈外静脉行PICC,对照组274例选择肘部静脉行PICC,置管后比较两组血栓及感染率。结果试验组血栓发生率为5.15%,对照组为22.63%,两组置管患者血栓发生率比较差异有统计学意义(P<0.05);试验组感染率为2.1%、对照组为4.0%,两组置管患者感染率比较差异无统计学意义。结论颈外静脉PICC较肘部静脉PICC血栓发生率低,置管成功率高;两组置管患者导管相关性感染无明显差异;颈外静脉PICC值得临床推广使用。
OBJECTIVE To compare the external jugular vein inserted peripherally inserted central catheter (PICC) with elbow vein inserted PICC-associated thrombosis and infections and explore the advantages and disadvantages of puncture for insertion of central venous catheter so as to provide guidance for optimal PICC catheterization . METHODS A total of 604 patients who underwent PICC in department of oncology from Jan 2013 to Jan 2014 were enrolled in the study and divided into two groups according to the catheterization sites ,330 cases in the experimen-tal group underwent the external jugular vein PICC ,while 274 cases in the control group received the elbow vein PICC ,and the incidence of thrombosis or infections was observed and compared between the two groups after the catheterization .RESULTS The incidence of thrombosis was 5 .15% in the experimental group ,22 .63% in the con-trol group ,and there was significant difference in the incidence of thrombosis between the two groups (P&lt;0 .05);the incidence of infections was 2 .1% in the experimental group ,4 .0% in the control group ,and there was no sig-nificant difference in the incidence of infections between the two groups .CONCLUSION The incidence of external jugular vein PICC-associated thrombosis is lower than that of elbow vein PICC-associated thrombosis ,with the success rate of catheterization high .There is no significant difference in the incidence of catheter-associated infec-tions .The external jugular vein PICC is worthy to be promoted in the hospital .
