摘要
目的总结脑室内、外中枢神经细胞瘤的影像学特征。方法回顾性分析13例经手术病理证实的脑室内、外中枢神经细胞瘤MRI表现,观察、分析肿瘤好发性别、年龄,好发部位,肿瘤边界、大小,肿瘤密度及信号特点,囊变、血管流空、强化模式及免疫组织化学等指标。结果 13例患者平均年龄31.5岁(21~55岁),男(n=7)、女(n=6)发病无明显差异,病变分别位于侧脑室(n=10)、椎管(n=2)、鞍区(n=1),多边界清晰(84.62%,11/13),体积较大,MRI平扫T1WI以等(n=7)或稍低(n=4)信号为主,T2WI以等(n=7)或稍高(n=4)信号为主;11例行DWI均呈稍高或高信号,肿瘤实体内可见囊变(84.62%,11/13)、血管流空(69.23%,9/13)信号,矢状位可见"扇形征"(61.54%,8/13),MRI增强扫描肿瘤呈轻度(76.92%,10/13)或明显强化(23.08%,3/13)。11例行免疫组织化学提示Syn阳性率约90.91%(10/11)。结论中枢神经细胞瘤好发于年轻人侧脑室孟氏孔周围,典型者有如下影像特征:(1)肿瘤体积较大,边界清晰;(2)T1WI呈等或稍低信号,T2WI呈等或稍高信号,DWI呈稍高或高信号;(3)瘤体内见囊变、血管流空影,矢状位可见"扇形征";(4)增强扫描肿瘤多呈轻度强化。肿瘤异位或信号不典型的中枢神经细胞瘤相对少见,影像表现尚缺乏统一认识,术前确诊有困难。
Objective To discuss imaging features of central neurocytomas, and to make a further understanding of the disease. Methods Thirteen patients with pathologically-proved central neruocytomas (CNC) were enrolled in this study. MRI findings were analyzed. The sex, age and location of predilection, the border, size and density of the tumor, and the MRI signal features, cystic degeneration, flow void signal, enhancement pattern and immunohistochemistry results were assessed. Results The patients included seven males and six females, and the mean age when the diagnosis was made was 31.5 years ( ranged from 21 to 55 year). The locations of the tumors included the lateral ventricle ( n = 10) , spinal canal( n = 2) and saddle area ( n = 1 ). The lesion usually had a clear bounder and a big size. Most tumors presented as isointensity or slight hypointensity signal on T1WI and isointensity or slight hyperintensity signal on T2WI. Shght or hyper-intensity signal was seen in all 11 cases who received DWI. MRI manifestations within the solid tumor included cystic degenera- tion (84.62% , 11/13), flow void effect (69.23%, 9/13) and scalloping appearance (61.54% , 8/13 ). On enhanced MRI, the lesions showed slight (76.92%, 10/13 ) or marked (23.08%, 3/13 ) inhomogeneous enhancement. Immunohistochemistry examination was employed in 11 cases, and positive Syn and GFAP was seen in 10 (90.91%) and 4 cases (36.36%) respectively. Conclusion CNC is typically located in the lateral ventricles around the foramen of Monro in young adults. The imaging characteristics of CNC are as follows: tumors usually having a sharp border and a big size, showing isointensity or slight hypointensity signal on T1WI while isointensity or slight hyperintensity signal on T2WI, and displaying slight or hyper-intensity signal on DWI. Cystic degeneration, flow void effect and scalloping appearance can be seen within the tumor. Most lesions present slight or marked enhancement after injection of contrast agent. C
