摘要
目的检测糖尿病视网膜病变(DR)患者外周血中的VEGF和IGF-1水平,并探讨其临床意义。方法选取30例健康者作为对照组,2型糖尿病患者120例为试验组;试验组再分为无糖尿病视网膜病变(NDR)组30例,单纯型糖尿病视网膜病变(BDR)组33例,单纯型糖尿病视网膜病变合并黄斑水肿(macular edema ME)组22例,增生型糖尿病视网膜病变(PDR)组35例。用ELISA法检测血浆VEGF和IGF-1水平。结果2型糖尿病患者外周血中VEGF及IGF-1水平明显高于正常者,差异有统计学意义(P<0.01)。而在2型糖尿病患者中,单纯型DR组血清VEGF及IGF-1水平明显高于无DR组,差异有统计学意义(P<0.05);单纯型并黄斑水肿组及增生型组血清VEGF水平明显高于单纯型DR组,差异有统计学意义(P<0.01),而血清IGF-1水平明显高于单纯型DR组,差异有统计学意义(P<0.05);而单纯型并黄斑水肿组和增生型组血清VEGF及IGF-1水平差异无统计学意义(P>0.05)。结论 2型糖尿病患者血清中VEGF及IGF-1水平随着DR的发生、黄斑水肿以及DR的加重而逐渐升高,对其进行监测有利于糖尿病视网膜病变的早期诊断和治疗。
Objective To detect the concentration of VEGF and IGF-1 in blood of patients with type 2 diabetic retinopathy, and to explore its clinical significance. Methods A total of 30 healthy person as normal control group, and 120 patients with type 2 diabetic retinopathy as experiment group. The cases of experiment group were divided into non-diabetic retinopathy (NDR) group (30 patients), background diabetic retinopathy (BDR) group (33 patients), background diabetic retinopathy combined with macular edema (BDR Combined with ME) group (22 patients) and proliferative diabetic retinopathy (PDR) group (35 patients). The levels of serum VEGF and IGF-1 were assayed by ELISA. Results The levels of serum VEGF and IGF-1 were significantly higher in DR group than those in healthy person (P〈0.01). And in the DR group, the levels of serum VEGF and IGF-1 were significantly higher in BDR group than those in NDR group (P〈0.05), the levels of serum VEGF was significantly higher both in BDR Combined with ME group and in PDR group than those in BDR group (P〈0.01), the levels of serum IGF-1 was significantly higher both in BDR combined with ME group (P〈O.05) and in PDR group (P〈0.05) than those in BDR group, the levels of serum VEGF and IGF-1 in PDR group and those in BDR combined with ME group had no difference(P〉0.05).Conclusion The levels of serum VEGF and IGF-1 increased when combining with macular edema, and increased during the development of DR. To detect the concentration of VEGF and IGF-1 in blood of patients with type 2 diabetic retinopathy maybe play a key role in the early diagnosis and in the treatment.
