摘要
目的探讨腹腔镜治疗良性卵巢囊肿的临床应用价值。方法将108例卵巢囊肿患者随机分为观察组和对照组,其中对照组实施常规开腹手术,观察组实施腹腔镜手术,对比分析两组患者的手术时间、平均出血量、术后排气时间、术后活动时间及术后住院时间;同时,对两组患者进行随访观察,分析两组患者术后1年内卵巢功能变化情况。结果观察组患者术中平均出血量明显少于对照组(P<0.05),在术后排气时间、活动时间及住院时间方面观察组亦明显短于对照组(P<0.05),而在手术时间方面未见明显差异(P>0.05);在术后卵巢功能方面,观察组术后第3、6及12个月时的卵巢功能较治疗前比较无明显差异,对照组术后的卵巢功能在第3、6个月较治疗前明显降低,且与同时期观察组患者的卵巢功能比较存在差异(P<0.05),在第12个月时恢复至术前水平。结论腹腔镜治疗卵巢囊肿临床效果确切,具有出血量较少、术后排气快、住院时间短的优势,且对卵巢功能的影响较小,适合无明显禁忌证的良性卵巢囊肿患者,值得推广应用。
Objective To explore the value of laparoscopic in the treatment of ovarian cyst. Methods All 108 patients were randomly divided into the observation group and the control group. The control group were treated by laparotomy,and the observation group were treated by laparoscopic surgery. The operative time,intraoperative blood loss,postoperative exhaust time,postoperative activity time,postoperative hospital stay as well as the ovarian function within a year of two groups were compared. Results The intraoperative blood loss in observation group was lower than control group（P〈0.05）.The postoperative exhaust time,postoperative activity time and postoperative hospital stay in observation group were significantly shorter than control group （P〈0.05）,but there was no difference in the operative time of two groups（P〉0.05）. There were no obvious changes in observation group with ovarian function,and the ovarian function in control group was abnormity in the 3 and 6 months after surgery （P〈0.05）,and there were back to normal until 12 months later. Conclusion Laparoscopic surgery with less bleeding,rapid postoperative recovery,shorter hospital stay,smaller influence for ovarian function and it is suitable for the patients with ovarian cysts without contraindication,itrs worthy of promotion and applicaction.
