摘要
目的查找医院门诊患者退药原因,探讨如何减少退药现象。方法收集医院门诊药房2012年5月至2013年4月退药371起,统计和分析退药科室、所退药品的类别和退药原因,并有针对性地采取干预措施。结果退药处方率为0.22%,患者自己要求退药占最大比例,检查取消占第2位。对造成80%责任的前6项因素实施干预,常态化后退药事件大大减少。结论医院发生的退药事件有一定的地域特点,通过提高患者用药的服务水平,与临床加强沟通,加强培训和退药事件的干预等措施可减少退药事件发生。
Objective To seek the causes of drug return among the outpatients in our hospital and to investigate how to reduce the phenomenon of drugs return. Methods 371 cases of drug return in the outpatient pharmacy of our hospital from May 2012 to April 2013 were collected and performed the statistical analysis on the drug return involved departments, kinds and causes of drug return,and the pertinent intervention measures were adopted. Results The ratio of drug return prescriptions was 0. 22% . Patients themselves requir-ing drug return accounted for the largest proportion and the second predominant cause was the check was cancelled. The top 6 factors responsible for about 80% of all responsibility were implemented by intervention. After normalization, the drug return events were greatly reduced. Conclusion The drug return events occurred in our hospital have certain regional characteristics. The intervention measure of increasing the service level for patient's medication, strengthening the communication with clinic and reinforcing training and intervention measures of the drug return events can reduce the occurrence of drugs return events.
