摘要
目的探讨C-反应蛋白(CRP)在老年呼吸系统感染性疾病中的诊断价值,以便为临床治疗提供一定的参考依据。方法选取医院2009年1月-2012年12月收治的老年呼吸系统感染患者140例,其中细菌性感染组70例,非细菌感染组70例,并对同期进行健康体检人员70名作为对照研究;观察患者血清CRP浓度、红细胞沉降率(ESR)、白细胞(WBC)计数、中性粒细胞(N)比值结果,并与正常组对照,同时对感染患者进行痰液培养,所有数据采用SPSS13.0软件进行统计分析。结果老年呼吸系统感染的患者血清中CRP、WBC、ESR及N比值分别为(92.23±32.37)mg/L、(11.32±3.13)×109/L、(19.81±10.76)mm/h及(72.31±10.12)%均高于正常组;且CRP浓度老年呼吸系统感染细菌组为(92.23±32.37)mg/L,显著高于非细菌组的(7.14±2.43)mg/L及正常对照组的(6.13±2.77)mg/L;细菌感染组CRP阳性率较WBC、ESR、N和痰培养阳性率高。结论 CRP检测在老年呼吸系统感染性疾病的诊断和鉴别诊断中具有敏感、快捷的特点,优于WBC、N,可作为老年人呼吸系统感染诊断、病情监测、疗效评估和预后判断的敏感指标,具有很高的临床应用价值。
OBJECTIVE To discuss the value of C-reactive protein(CRP)in diagnosis of elderly patients with respiratory infections in order to provide some reference for clinical treatment.METHODS A total of 140 elderly patients with respiratory infections from Jan.2009 to Dec.2012 in our hospital were studied.Of 140 patients,there were 70 patients with bacterial infections which were set as the infection group,and 70non-infected patients set as the non-infection group.And 70 healthy people who came to our hospital for physical examination during the same period were chosen as the control group.CRP concentration,erythrocyte sedimentation rate(ESR),white blood cell count(WBC),neutrophil ratio(N)were observed in the observation group and compared with the normal group.Meanwhile,sputum culture was conducted for infected patients.All data were analyzed with SPSS13.0.RESULTS Serum CRP,WBC,ESR and neutrophil ratios were(92.23±32.37)mg/L,(11.32±3.13)×109/L,(19.81±10.76)mm/h,and(72.31±10.12)% in elderly patients with respiratory infections and were higher than in the normal group.And the concentration of CRP was(92.23±32.37)mg/L in the elderly group with bacterial respiratory infections,which was significantly higher than(7.14±2.43)mg/L in the non-infection group and(6.13±2.77)mg/L in the normal control group.In the bacterial infection group,the positive rate of CRP was higher when compared with that of WBC,ESR,N and sputum culture.CONCLUSION CRP testing in diagnosis and differential diagnosis of elderly respiratory infectious diseases is sensitive and fast,better than WBC and N tests,which can be used as the sensitive index for diagnosis,disease monitoring,efficacy assessment and prognosis judgment for elderly respiratory infection,with a high clinical value.
