摘要
目的分析先天性心脏病合并下呼吸道感染患儿痰培养阳性率、病原菌分布及药敏试验结果,为临床防治感染提供选药依据。方法选择我院2008年7月～2011年6月住院的先天性心脏病合并下呼吸道感染患儿63例,对其痰液进行培养和药敏试验。结果 63例标本痰液培养阳性46例,阳性率73.02%。检出菌种11种,革兰阴性菌(G-菌)6种,革兰阳性菌(G+菌)5种。阳性标本共分离出细菌53株,其中G-菌37株,占69.81%,G+菌16株,占30.19%。革兰阴性菌对左氧氟沙星、环丙沙星、阿米卡星、头孢他啶、头孢唑肟、头孢哌酮/舒巴坦、亚胺培南敏感率较高,对头孢曲松、头孢噻肟中度敏感,对阿奇霉素、复方新诺明耐药。革兰阳性菌对左氧氟沙星、环丙沙星、阿米卡星、万古霉素、阿奇霉素敏感,对头孢曲松、头孢他啶、头孢唑肟、亚胺培南、利福平中度敏感,对头孢噻肟、头孢哌酮/舒巴坦、复方新诺明耐药。结论先天性心脏病合并下呼吸道感染患儿的痰液培养病原菌种类多,耐药菌株多,临床用药限制多,应选择对致病菌敏感的抗生素为主,痰培养结果指导临床合理用药非常必要。
Objective To analyze the positive results in bacteria culture , distribution and drug resistance of pathogens in children with congenital heart disease and lower respiratory tract infection. Methods 63 children with congenital heart disease and lower respiratory tract infection were selected and the results of sputum cultures and drug sensitive test were analyzed. Results In 63 specimens, 46 sputum cultures were positive,and the positive rate was 73.02%.11 pathogens were proven. 6 were gram-negative bacteria , Mainly Haemopbilus parainfluenzae, Klebsiella pneumoniae, Escherichia coli-based ,5 were gram-positive bacteria. Mainly Staphylococcus aureus, Streptococcus pneumoniae based, 53 strains of bacteria were isolated from 46 positive samples, with 37 （69.81%） strains of gram-negative bacteria and 16 gram-positive bacteria （30.19%）. Conclusion There are many kinds of strains in sputum culture , more drug-resistant strains and restrictions for drug use in children with congenital heart disease and lower respiratory tract infection.It is necessary for sputum culture to provide important information to choose reasonable antibiotics.
