摘要
目的探讨阿托伐他汀对单纯收缩期高血压血脂正常患者颈动脉内中膜厚度(IMT)的影响。方法选择单纯性收缩期高血压血脂正常患者150例,随机分为治疗组(n=75)和对照组(n=75)。对照组患者给予口服苯磺酸氨氯地平(络活喜,5 mg,1/日)降压治疗,治疗组在对照组降压药物基础上,加用阿托伐他汀(立普妥,20 mg,1/晚)治疗,干预疗程为12个月。采用高频超声观察两组患者治疗前、后的颈总动脉(CCA)、颈动脉膨大处(BULB)、颈内动脉(ICA)IMT厚度及血压指标变化。结果两组患者治疗前颈动脉CCA、BULB和ICA的IMT值及血压指标均无统计学差异(P均>0.05)。药物干预12月后,治疗组患者CCA、BULB、ICA的IMT厚度分别为0.80±0.17 mm、0.91±0.18 mm、0.67±0.18 mm;对照组患者CCA、BULB、ICA的IMT厚度分别为0.88±0.21 mm、1.07±0.18 mm、0.76±0.19mm;与对照组比较,治疗组患者干预后CCA、BULB和ICA的IMT值明显下降,差异有统计学意义(P<0.05)。两组患者治疗后,治疗组与对照组患者脉压差分别为58.06±5.56 mmHg、67.18±4.68 mmHg,差异有统计学意义(P<0.05)。结论应用阿托伐他汀能够改善单纯收缩期高血压血脂正常患者颈动脉内中膜厚度,降低脉压差水平,对延缓动脉硬化进展有帮助。
Objective To discuss the influence of atorvastatin on carotid artery intima-media thickness (CA-IMT) in the patients with isolated systolic hypertension and normal blood fat. Methods The patients (n=150) were randomly divided into treatment group and control group (each n=75), and control group was orally given Norvasc (5 mg once a day) and treatment group was given Lipitor (20 mg once a night) and Norvasc for 12 m. The changes of the IMT of CCA, BULB and ICA and blood pressure (BP) indexes were observed by using high-frequency ultrasound in 2 groups before and after treatment. Results Before treatment the IMT of CCA, BULB and ICA and BP indexes had no statistical difference between 2 groups (all P〉0.05). After treatment for 12 m, the IMT of CCA, BULB and ICA was, respectively, 0.80±0.17 mm, 0.91±0.18 mm and 0.67±0.18 mm in treatment group, and 0.88 ±0.21 mm, 1.07±0.18 mm and 0.76±0.19 mm in control group. Compared with control group, the IMT of CCA, BULB and ICA decreased significantly in treatment group (P〈0.05). After treatment, pulse pressure was 58.06±5.56 mmHg in treatment group and 67.18±4.68 mmHg in control group (P〈0.05). Conclusion The administration of Lipitor can ameliorate CA-IMT, reduce pulse pressure and delay the progress of atherosclerosis in the patients with isolated systolic hypertension and normal blood fat.
