摘要
目的 2型糖尿病患者较一般人群易感染幽门螺杆菌,而且根除率远低于后者,此次研究探索以呋喃唑酮为三联根除方案对糖尿病患者感染H.pylori初次治疗的疗效的比较,统计其中胃轻瘫患者缓解率。方法将112例经查14C呼气试验证实的H.pylori阳性的2型糖尿病胃轻瘫痪患者分为三组,其中A组及C组给予左氧氟沙星+呋喃唑酮+埃索美拉唑疗程(三联10d疗法),并且A组加用多潘立酮口服10 d,而B组只使用多潘立酮口服10 d,结束4周后行14C-尿素呼气试验(14C-UBT)检测,观察A、C组H.pylori根除率,胃排空试验检测根除前后糖尿病胃轻瘫患者排空时间变化。结果 C组幽门螺杆菌根除率达86.36%(47/52),而A组根除率为69.64%(38/56),两组有统计学差异(P<0.05)。A组成功根除幽门螺杆菌后胃轻瘫症状缓解率为78.9%(30/38),而B组胃轻瘫症状缓解率为53.6%(30/56),差异有统计学意义(P<0.05)。结论以呋喃唑酮为三联的10 d疗法具有较高的根除率,但在糖尿病患者中疗效相对较差,根除幽门螺杆菌可以明显改善糖尿病胃轻瘫症状。
Objective Type 2 diabetes melhius were more easily infected with H.pylori than the general population, and eradication rate is far lower than the latter, this study explored with furazolidone as the triple eradication program for diabetic patients infected with H. pylori treatment for the first time, statistics of gastroparesis patients with remis- sion rate. Methods All 112 patients with type 2 diabetes gastroparesis and 110 patients with common infected with H. pylori, confirmed by 14C breath test of H. pylori positive, were divided into three groups. A group（56） were given lev- ofloxacin ＋ furazolidone ＋ esomeprazole ＋ domperidone（10 days）, C group（52） patients were given levofloxacin ＋ fu- razolidone ＋ esomeprazole （10 days）, B group （56） were only given domperidone （10 days）. 4 weeks later,14C UBT were detected H.p ylori eradication. Gastric emptying test were obserevded the emptying timeof diabetic gastroparesis patients before and after h. pylori edradicated. Results Type 2 diabetes melltius eradication rate was 69.64% （38/56）A group,the C group eradication rate was 86.36%（47/52）, statistical differences were founded in the two groups （P〈 0.05）. The remission rate of gastroparesis in the successful eradication of helicobacter pylori （group A） was 78.9% （30/ 38）, however, the gastroparesis symptom relief rate of group B was 53.6% （30/56）（P〈0.05）. Conclusion With fura- zolidone as the triple therapy has high eradication rate in ten days, but relatively poor curative effect in type 2 diabetes melltius. The eradication of helicobacter pylori can obviously improve the symptoms of gastroparesis.
