急诊经皮冠状动脉介入术中应用血栓抽吸导管及替罗非班的疗效与安全性分析被引量：7

摘要目的探讨急性ST段抬高型心肌梗死(STEMI)患者急诊经皮冠状动脉介入(PCI)应用血栓抽吸导管及替罗非班的疗效及安全性。方法回顾性分析行急诊PCI的STEMI患者197例,分为直接PCI组(A组)、PCI+血栓抽吸或替罗非班组(B组)、PCI+血栓抽吸+替罗非班组(C组),比较各组术后TIMIⅢ级血流、慢血流或无复流、TIMI血流帧数(TFC)、心肌呈色分级(MBG)、2 h内ST段回落率、术后1 w周及3月后左室射血分数(LVEF)和左室舒张末内径(LVEDd)、大出血情况、住院期间主要心血管不良事件(MACE)等指标的差异。结果三组术后TIMIⅢ级血流、慢血流或无复流、TFC、MBG、2 h内ST段回落率比较差异有统计学意义(P<0.05);三组住院期间大出血情况、MACE以及术后1 w LVEF、LVEDd比较差异无统计学意义(P>0.05),但随访3月后三组LVEF、LVEDd比较差异有统计学意义(P<0.05)。结论联合应用血栓抽吸导管和替罗非班能有效减轻STEMI患者冠脉内血栓,改善心肌组织灌注和术后心功能。
