目的分析北京市海淀区2012年疑似预防接种异常反应(adverse events following immunization,AEFI)的发生特征,评价海淀区AEFI监测系统运转情况。方法通过AEFI信息管理系统,收集2012年报告的AEFI,采用描述性方法进行流行病学分析。结果 2012年海淀区共报告AEFI个案174例,报告发生率12.0/10万,一般反应报告率为6.4/10万,48 h内报告率和调查率均为100%。AEFI报告个案中≤2岁占82.2%,男、女比例为1.24:1,多发生在接种后24 h,没有明显的季节聚集性。反应分类一般反应占53.5%,异常反应占31.0%,偶合症占15.5%。一般反应以接种百白破后发热/红肿/硬结发生率最高(38.2/10万),异常反应以接种7价肺炎疫苗过敏性皮疹发生率最高(53.3/10万)。结论海淀区AEFI监测敏感性和及时性较高,但仍需进一步加强一般反应监测。
Objective To analyze the occurrence features of the suspected adverse events following immunization (AEFI) in Haidian District, Beijing City in 2012, and to evaluate the implementation running status of AEFI surveillance system. Meth- ods The data of AEFI cases in 2012 were collected from the national AEFI information system, and the descriptive methodolo- gy was employed to conduct epidemiologieal analysis. Results A total of 174 AEFI cases were reported in Haidian District in 2012, with the reported incidence rate and common reaction incidence rate being 12.0/100,000 and 6.4/100,000, respectively. The rates of reporting and investigation within 48 h were both 100%. 82.2% of the AEFI cases were children aged 2 yearn or less, and the ratio of male to female cases was 1.24 : 1. The vaccine reactions often occurred within the first 24 - hour post - vac- cination, without obvious clustering in seasonal distribution. The classification diagnoses of the AEFI cases showed that common and minor reactions, abnormal reactions and coincidental illness accounted for 53.5%, 31.0% and 15.5%, respectively. Among common and minor reactions, the incidence rate of DTaP - induced fever, local swelling and scleroma was the highest (38.2/ 100,000 ). Among abnormal reactions, the incidence rate of PPCV7 - induced anaphylactic rash was the highest (53.3/ 100,000). Conclusions The surveillance of AEFI eases in Haidian District is sensitive and timely, but it is still necessary to intensify the monitoring of common reactions and post - vaccination care.
