丙泊酚和咪唑安定用于ICU呼吸机治疗病人的镇静 被引量:37

摘要 目的 观察丙泊酚和咪唑安定对ICU呼吸机治疗病人的镇静效果 ,为危重病人镇静用药提供依据。方法 选择在ICU呼吸机治疗的病人 40例 ,随机分为两组。丙泊酚组 2 1例 ,先静注丙泊酚 1 5mg/kg镇静诱导 ,然后改用微量注射泵持续注射丙泊酚 ,根据Ramsay氏分级标准调整用量 ;咪唑安定组 19例 ,先静注咪唑安定 0 0 5mg/kg镇静诱导 ,然后改用微量注射泵持续注射咪唑安定 ,根据不同镇静分级调整用量。分别记录Ramsay氏分级Ⅲ~Ⅵ级时的用药量及用药前后、苏醒时的心率、血压、脉搏血氧饱和度和停药后苏醒时间 ,停药后再入睡和恶心发生情况。结果 两组病人随镇静程度的加深而用药量增加 ,但呼吸、循环无明显抑制。丙泊酚组较咪唑安定组苏醒时间快 ,且苏醒后再入睡及出现恶心病人明显减少。结论 丙泊酚和咪唑安定都能达到ICU病人所需的镇静要求。丙泊酚苏醒快 。 Objective To observe sedative effect of propofol and midazolam used in mechanically ventilating patients in the intensive care unit(ICU).Methods Fourty patiens with mechanical ventilation in ICU were randomly divided into two groups.The patients in propofol group were injected intravenously with propofol 1.5mg/kg as a loading dose,and infused continuously with a pump thereafter.The dose infused was adjusted according to Ramsay classification standard.The patients of midazolam group were injected intravenously with midazolam 0.1mg/kg as a loading dose,and infused continuously as in propofol group.The dosage,heart rate,blood pressure,arterial oxygen saturation before and after using the drugs and recovery time were recorded when grade Ⅲ-Ⅵ of Ramsay classification were reached.The recovery time after stopping infusion of the drugs as well as re-sleep and emesis were also recorded.Results The dosage of the two groups increased with the increase of sedation degree.The respiratory and circulation parameters were stable.The recovery time of propofol group was shorter than that of midazolam group.The number of patients sleeping again after recovery and emesis decreased obviously in propofol group.Conclusion Both propofol and midazolam can reach the goal of sedation needed for ICU patients.Propofol can lead to faster recovery after stopping the drug.The incidence of re-sleep after recovery and emesis is less when propofol is used for ICU patient's sedation.
