摘要
目的探讨骨调素(OPN)、趋化因子和细胞因子水平与银屑病发生的关系,同时分析银屑病患者上述因子水平与患者肥胖状态的关系。方法纳入2011年3月—2015年6月银屑病患者108例和健康志愿者31例,统计分析银屑病患者和健康人以及不同程度肥胖患者OPN、趋化因子和细胞因子水平的差异性。结果研究组OPN水平显著高于对照组(t=8.399,P <0.001),但是研究组中肥胖患者和非肥胖患者并无显著差异(t=0.114,P=0.455);OPN诊断银屑病患者的临界值为18.01 ng/ml,诊断的敏感性为78%,特异性为74%;研究组CC类趋化因子配体5(CCL5)、趋化因子(C-X-C)配体9(CXCL9)、肿瘤坏死因子-α(TNF-α)和γ-干扰素(IFN-γ)水平均高于对照组,且差异有统计学意义(t=7.641,P <0.001;t=2.757,P=0.003;t=10.067,P <0.001;t=3.683,P=0.011);研究组OPN水平与CXCL9和TNF-α水平均无显著相关性(r=0.008,P=902,r=0.112,P=887)。结论银屑病患者OPN、CCL5和CXCL9均高于健康者,其水平不受肥胖影响,OPN可能是影响银屑病发病机制的重要分子,可作为银屑病诊断的生物标志物,并且不依赖于CXCL9和TNF-α水平。
Objective To analysis the relationship among osteopontin(OPN), chemokines, cytokines level and psoriasis, moreover analysis the correlation between factors above-mentioned and obesity. Methods A total of 108 patients with psoriasis and 31 healthy volunteers were included from March 2011 to June 2015. Statistical analysis was used to compare the differences in the levels of osteopontin, chemokines, and cytokines between patients with psoriasis and healthy subjects, as well as different levels of obesity. Results The OPN levels of research group were significantly higher than the control group(t=8.399, P<0.001), but there was no significant difference between obesity and non-obesity patients in research group(t=0.114, P=0.455). The critical value of OPN for psoriatic patients diagnosis was 18.01 ng/ml, the sensitivity of the diagnosis was 78%, the specificity of the diagnosis was 74%. The levels of CCL5, CXCL9, TNF-α and IFN-γ of research group were significant higher than control group(t=7.641, P<0.001;t=0.001, P=0.003;t=10.067, P<0.001;t=3.683, P=0.011). There was no significant correlation between OPN level and CXCL9 and TNF-α levels in the study group(r=0.008, P=902;r=0.112, P=887). Conclusion In patients with psoriasis, the level of OPN, CCL5 and CXCL9 were all higher than healthy volunteers, and the levels were not affected by obesity. OPN may be an important factor for the pathogenesis of psoriasis, which can be used as a diagnostic biomarker of psoriasis, and independent of TNF-α and CXCL9 levels.
