摘要
目的比较静脉腔内激光介入术与传统剥脱术治疗CEAP 3～4级下肢静脉曲张的临床效果。方法选取乐山市人民医院血管外科2016年2月—2017年11月收治的CEAP 3～4级下肢静脉曲张患者86例,随机分为传统组42例和激光组44例。传统组患者采用传统剥脱术治疗,激光组患者采用静脉腔内激光介入术治疗。比较两组患者术中出血量、切口长度、术后24 h视觉模拟评分法(VAS)评分、疼痛持续时间、卧床时间、患肢功能恢复时间、住院时间、术后并发症及术后30 d症状完全消失情况。结果激光组患者术中出血量少于传统组,切口长度小于传统组,术后24 h VAS评分低于传统组,疼痛持续时间、卧床时间、患肢功能恢复时间及住院时间短于传统组(P<0.05)。两组患者术后并发症发生率、术后30 d症状完全消失率比较,差异无统计意义(P>0.05)。结论静脉腔内激光介入术治疗CEAP 3～4级下肢静脉曲张的临床效果优于传统剥脱术,其可更有效促进患者康复,且安全性高。
Objective To compare the therapeutic efficacy of intravenous laser intervention and traditional stripping surgery in treating CEAP 3-4 of varicose vein of lower extremity.Methods A total of 86 cases of CEAP 3-4 of varicose vein of lower extremity patients were selected from February 2016 to November 2017 in Leshan People’s Hospital,which were randomly divided into traditional group(n=42) and laser group(n=44).The traditional group was treated with traditional stripping surgery,laser group was treated with intravenous laser intervention.The amount of bleeding,length of incision,postoperative 24 h VAS scores,duration of pain,duration of bed,recovery time,length of stady,the incidence of comlications and complete disappearance of postoperative 30-day symptoms were compared between the two groups.Results The laser group of amount of bleeding was less than traditional group,length of incision was smaller than traditional group,postoperative 24 h VAS scores were lower than traditional group,duration of pain,duration of bed,recovery time and length of stady were shorter than traditional group(P<0.05).There was no significant differences of the incidence rate of comlications or complete disappearance rate of postoperative 30-day symptoms between the two groups(P>0.05).Conclusion Intravenous laser intervention has a better effect in treating CEAP 3-4 of varicose vein of lower extremity than traditional stripping surgery,it can more effectively promote the rehabilitation of patients,and with high safety.
