摘要
刘茂才教授认为,多系统萎缩为本虚标实之沉疴顽疾,治疗当分清虚实,不可执其一端。对以帕金森症状为主的MSA-P型从颤拘病进行论治,认为病机属肝肾亏损、气血两虚,兼有肝阳上亢化风、风动扰痰、痰瘀互结、阻滞经络;治当补益肝肾气血,兼以滋阴潜阳、熄风止颤。对以小脑共济失调症状为主的MSA-C型应从骨摇辨治,病机属气血肝肾亏虚、痰瘀阻滞经络,且以阳虚为主;临证应大补气血,温补肾阳,兼以温阳运脾、活血涤痰通络。临证施药宜重用黄芪,固守宗法,缓图治之,以求全效。
According to Professor LIU Maocai,multiple system atrophy is such a kind of severe and lingering disease with deficiency in root and excess in branch,it is required to differentiate deficiency and excess without partiality. MSA-P mainly characterized by Parkinsonian symptoms is treated from the tremor-rigidity disease, since it is believed that its pathogenesis falls into liver-kidney deficiency and qi-blood deficiency,along with hyperactivity of liver yang transforming into wind, wind-disturbance due to stirring wind, intermingled phlegm and stasis and stagnation of meridian and collateral.Therefore,emphasis of the treatment should be placed on tonifying liver,kidney,qi and blood in addition to nourishing yin with suppressing hyperactive yang and stopping tremor by ceasing wind. MSA-C mainly characterized by cerebellar ataxia symptoms is treated on the basis of bone shaking,since it is believed that its pathogenesis falls into liver-kidney-qi-blood deficiency and phlegm and stasis stagnating meridian and collateral,among which yang deficiency is dominant. Therefore,clinical emphasis should be placed on strongly reinforcing qi and blood and warmly nourishing kidney yang in addition to warming yang and activating spleen,activating blood,dispelling phlegm and unblocking collateral. In clinical practice,the use of astragalus should be focused on and the fundamental principles should be conformed to,thus to expect the optimal clinical effect.
