术前细针穿刺细胞学的分子检测在甲状腺癌诊断和治疗中的研究进展 被引量:12

Preoperative molecular testing on fine-needle aspiration in precision management of thyroid cancer

摘要 本文围绕甲状腺癌诊治中有争议的热点问题,系统介绍了目前国内外几种常用的分子检测方法,特别是基因突变及融合检测,在甲状腺结节的术前诊断和甲状腺癌的预后评估中的重要意义。对于穿刺细胞学诊断不确定的甲状腺结节,不同的分子检测方法各有其优点和不足,其中DNA检测,特别是基于二代测序技术的多基因联合检测具有较好的灵敏度和特异度。除了用于诊断,它还可以检出与肿瘤预后有关的基因变异,如BRAF^V600E突变和TERT启动子突变等,具有较广阔的应用前景。 This article focuses on the controversial issues in the diagnosis and treatment of thyroid cancer.Several commonly used molecular tests,especially gent mutation and fusion detection,in the preoperative diagnosis of thyroid nodules and the prognosis evaluation of thyroid cancer are introduced.For the thyroid nodules with uncertain indeterminate cytology,different molecular screening methods have their own advantages and disadvantages.Among them,DNA detection,especially muhi-gene molecular testing based on next-generation sequencing has better sensitivity and specificity.Besides being used in diagnosis, it can also detect gene mutations related to tumor prognosis,such as BRAF^V600E mutation and TERT promoter mutation,which is increasingly widespread and accepted.
