摘要
目的探讨糖类抗原19-9(CA19-9)、癌胚抗原(CEA)和总胆红素(TB)联合检测在胰腺囊性肿瘤良恶性鉴别中的临床诊断与应用价值。方法选取2015年1月至2017年1月间陕西省宝鸡市人民医院收治的116例胰腺囊性肿瘤患者,分别行血清CA19-9、CEA和TB检测,以病理学检查结果进行分组,将106例恶性胰腺囊性肿瘤患者纳入恶性胰腺囊性肿瘤组; 10例良性胰腺囊性肿瘤患者纳入良性胰腺囊性肿瘤组。比较两组患者的肿瘤标志物水平,并计算CA19-9、CEA和TB单独检测与CA19-9、CEA和TB联合检测恶性胰腺囊性肿瘤的阳性率、敏感性、准确性和特异性。结果恶性囊胰腺囊性肿瘤患者血清CA19-9、CEA和TB水平均显著高于良性胰腺囊性肿瘤患者; CA19-9、CEA和TB单独检测恶性胰腺囊性肿瘤阳性率均显著低于三组联合检测的阳性率,差异均有统计学意义(均P <0. 05)。CA19-9、CEA和TB联合检测恶性胰腺囊性肿瘤的灵敏度与准确度均显著高于CA19-9、CEA和TB单独检测,差异均有统计学意义(均P <0. 05)。CA19-9、CEA和TB联合检测恶性胰腺囊性肿瘤的特异性与CA19-9、CEA和TB单独检测的特异性无显著差异,差异无统计学意义(P> 0. 05)。结论 CA19-9、CEA和TB联合诊断可显著提升恶性胰腺囊性肿瘤的确诊率,值得在胰腺囊性肿瘤良恶性的鉴别诊断中应用推广。
Objective To explore the clinical diagnostic value and clinical application value of carbohydrate antigen 19-9( CA19-9),carcinoembryonic antigen( CEA) and total bilirubin( TB) in distinguishing malignant from benign pancreatic cystic neoplasms. Method A total of 116 patients with pancreatic cystic tumors treated at Baoji People’s Hospital from January 2015 to January 2017 were selected. Serum detection of CA19-9,CEA and TB was performed. They were divided into groups according the pathological findings. One hundred and six patients with malignant pancreatic cystic neoplasms were assigned to malignant pancreatic cystic neoplasms group and 10 with benign pancreatic cystic neoplasms were assigned to benign pancreatic cystic neoplasms group. The levels of tumor markers were compared between the two groups.The positive rate,sensitivity,accuracy and specificity for malignant pancreatic cystic tumors were compared between CA19-9,CEA,TB single detection and combination detection. Results The serum levels of CA19-9,CEA and TB were significantly higher in patients with malignant cystic pancreatic tumors than in patients with benign cystic pancreatic tumors( all P < 0. 05). The positive rates of patients with malignant cystic pancreatic tumors were significantly lower for CA19-9,CEA and TB single detection than for the combined detection of three indicators( all P < 0. 05). The sensitivity and accuracy of CA19-9,CEA and TB combined detection were significantly higher than those of CA19-9,CEA and TB single detection( all P <0. 05). There was no significantly different in specificity between combined detection of CA19-9,CEA and TB and CA19-9,CEA and TB single detection for malignant pancreatic cystic tumors( P > 0. 05). Conclusion CA19-9,CEA and TB combined diagnosis can significantly improve the diagnostic rate for malignant pancreatic cystic tumors. It was worth popularizing in distinguishing malignant from benign pancreatic cystic neoplasms.
