摘要
目的探讨全模型迭代重组(IMR)联合低管电压(kV)技术在肠系膜上动脉(SMA)CT血管造影(CTA)中对降低辐射剂量及控制图像质量的应用价值。方法将60例行腹盆腔动态CT增强扫描的患者随机分为常规组(120 kV)和低剂量组(80kV),每组30例。根据重组算法将常规组分为A、B两组,低剂量组为C组。A组采用120 kV,i Dose^4重组;B组采用120 kV,IMR重组;C组采用80 kV,IMR重组,余参数均一致。常规组与低剂量组年龄、体质指数(BMI)、用碘总量、有效辐射剂量(ED)比较使用独立样本t检验;A、B、C三组间SMA血管强化CT值、噪声(SD)、信噪比(SNR)、对比噪声比(CNR)及SMA图像质量评分比较使用单因素方差分析。结果常规组与低剂量组的年龄、BMI和用碘总量无显著性差异(P >0.05)。常规组和低剂量组的ED分别为(7.2±1.6)m Sv和(2.3±0.5)m Sv,差异有统计学意义(P<0.01)。A、B、C三组的SMA血管强化CT值分别为(311.3±48.2)HU、(333.0±47.9)HU和(487.6±75.1)HU,A、B两组间差异无统计学意义(P >0.05),余组间差异均有统计学意义(P<0.01);三组SD值分别为(33.3±4.8)HU、(11.2±1.1)HU和(18.4±2.3)HU,三组间的差异均有统计学意义(P<0.01);三组CNR、SNR分别为(10.2±2.4)、(11.0±2.8),(30.2±5.7)、(32.2±7.6),(26.9±5.0)、(34.8±7.4),B、C两组间差异无统计学意义(P >0.05),余组间差异均有统计学意义(P<0.01);三组SMA-CTA图像质量评分分别为(2.4±0.4)分、(3.1±0.3)分和(3.6±0.3)分,三组间的差异均有统计学意义(P<0.01)。结论低kV-CT扫描联合IMR重组技术不仅显著降低CT辐射剂量,而且可以降低图像噪声,提高图像质量及SMA远端血管的显示率。
Objective To evaluate the application of iterative model reconstruction(IMR)combined with low tube voltage in superior mesenteric artery CT angiography(CTA)for reducing radiation dose and controlling image quality.Methods 60 cases of abdominal and pelvic enhancement CT scans were randomly divided into standard dose group and low dose group,standard dose group assigned to group A、B according to different reconstruction algorithms,low dose group regarded as group C.group A(n=30,120 kV and i Dose^4reconstruction),group B(n=30,120 kV and IMR reconstruction),group C(n=30,80 kV and IMR reconstruction),while the other parameters kept consistent.We compared the age,BMI,iodine consumption amount,effective dose(ED)of standard dose group and low dose group with independent-sample t test.We also compared the artery CT value,image noise(SD),signal noise ratio(SNR),contrast noise ratio(CNR)and superior mesenteric artery image quality with one-way analysis of variance.ResultsThe difference of age,BMI,iodine consumption quantity between standard dose group and low dose group had no statistic significance(P >0.05).The ED of standard dose group and low dose group were(7.2±1.6)m Sv,(2.3±0.5)m Sv,The difference of data had statistic significance(P<0.01).The CT value of SMA among group A,B and C were(311.3±48.2)HU,(333.0±47.9)HU and(487.6±75.1)HU,there had no statistic significance among group A and B(P >0.05),whereas the data difference for the rest groups had statistical significance(P<0.01).SD were(33.3±4.8)HU,(11.2±1.1)HU and(18.4±2.3)HU,The difference among the three groups had statistical significance(P<0.01).SNR、CNR were(10.2±2.4),(11.0±2.8);(30.2±5.7),(32.2±7.6);(26.9±5.0),(34.8±7.4);they had no statistic significance among group B and C(P >0.05),whereas the data difference for the rest of the groups had statistical significance(P<0.01).The CT angiography of superior mesenteric artery image quality were(2.4±0.4),(3.1±0.3),(3.6±0.3),the difference of data among three groups had statistical significance(P<0
