摘要
近年中国糖尿病患病率增长加快,疾病负担日益加重。中国于1995年制定糖尿病防治规划纲要,并先后出台相关政策,于2017年发布《中国防治慢性病中长期规划(2017-2025年)》,形成依托社区卫生服务中心、综合医院参与、社区-医院一体化、患者自我管理、疾病预防控制中心或卫计委组织参与的5种糖尿病防治模式,但存在糖尿病防治的重心未落实到糖尿病高危人群管理、基层糖尿病患者管理人员数量不足及专业技能偏低、医防未有效整合及工作机制运行不畅等问题。随着健康中国建设及医改的不断深入,应将健康融入所有政策中,整合综合医院、疾控机构及社区的各种资源,推进糖尿病分级诊疗,关注一般人群和高危人群,引进外资或第三方购买服务来弥补人力物力的不足,增设岗位并加强培训和考核,学习国外的成功经验,如设立专项基金和项目,开展个案管理与自我管理。通过坚持预防为主、推行健康文明的生活方式和营造绿色安全的健康环境来实现良好的健康治理。
The prevalence of diabetes in China has increased rapidly in recent years. The burden of disease is also getting worse. Since 1995,China has formulated the outline for the prevention and control of diabetes,then introduced relevant policies successively. China’s medium and long-term plan for the prevention and treatment of chronic diseases( 2017-2025) was released in 2017. Five diabetes prevention and treatment modes had formed it included relying on community health service center,comprehensive hospital participation,community hospital integration,patient self-management,CDC or health and family planning commission. But insufficient management of diabetes high-risk groups,insufficient management personnel of primary diabetes patients,low professional skills of management personnel,unintegrated of treatment and prevention had existed. With the continuous deepening of health China construction and medical reform,it should integrate health into all policies and integrate various resources of comprehensive hospitals,disease control institutions and communities. It should advance progression of diabetic grade diagnosis and treatment,concern the general population and the high-risk group,introduce foreign capital or third-party institutions,post to add and training assessment enhancement. What is more,it could earn the successful experience abroad. For example,special funds and projects are set up to carry out case management and selfmanagement. By insisting on prevention and promoting the healthy and civilized life style,it will create a green and safe healthy environment to achieve good health governance.
