自体软骨联合富血小板血浆移植修复兔关节骨软骨缺损的效果 被引量:3

The effects of autologous cartilage combined with platelet-rich plasma on the repairmen of osteochondral defects in rabbits

摘要 目的观察自体软骨联合自体富血小板血浆移植后对兔关节骨软骨缺损修复的效果,从而探索一种新的关节内软骨缺损的修复方法。方法采用新西兰大白兔构建关节软骨缺损模型,制备富血小板血浆,将软骨缺损的兔子随机分为3组。对照组1:将单纯自体骨软骨植入软骨缺损处;对照组2:将单纯富血小板血浆植入软骨缺损处;实验组:将自体富血小板血浆与自体骨软骨复合体植入软骨缺损处,每组10只。分别在术后2、4周处死动物取材,进行大体观察和HE染色、番红O固绿染色、Masson染色病理观察,评估关节软骨损伤的修复情况。结果术后活体观察2、4周实验组较对照组1、对照组2动物活动恢复得较快。组织大体观察发现,随着时间的延长,各组缺损软骨部位软骨增多,但实验组缺损处新生软骨较其他组生长更快且质地韧性、有弹性。组织染色观察发现对照组1移植软骨与周围骨质连接不紧密有间隙,对照组2软骨生长慢,软骨陷窝明显,实验组移植软骨与周围骨质融合效果好,软骨缺损恢复快,实验组的软骨缺损处恢复能力高于对照组1和对照组2,修复效果更佳。结论自体软骨联合富血小板血浆移植有效修复兔骨软骨缺损,促进了周围正常软骨高度整合,具有可行性。 Objective To observe the effect of autologous cartilage combined with autologous platelet-rich plasma (PRP)on the repairmen of osteochondral defects in rabbits,so as to explore a new method of repairing intra-articular cartilage defects.Methods Rabbit model of articular cartilage defect were established in New Zealand white rabbits. PRP were also prepared.Rabbits with cartilage defects were randomly divided into 3groups,Group Control 1(cartilage defects were implanted with pure osteochondral cartilage);Group Control 2(pure PRP was implanted into cartilage defects)and Group Experiment (autologous PRP and autologous osteochondral composite were embedded in cartilage defects).The animals were sacrificed 2 and 4 weeks after operation.Gross observation,HE staining,safranin O solid green staining and Masson staining were performed to evaluate the repairing of articular cartilage injury.Results At the 2 weeks and 4 weeks,compared with Group Control 1 and 2,animal activities significantly recovered faster in Group Experiment.With the extension of time,cartilage defects were partially recovered in each group,however,the newborn cartilage in defect area in experimental group was faster with more toughness and elasticity than the other 2 groups.Tissue staining showed that graft cartilage was not connected to the surrounding bone closely in Group Control 1,as there were gaps between them;and cartilage growth was slow and cartilage lacuna was obvious in Group Control 2.Transplantation of cartilage showed good fusion with surrounding bone and fast growth in Group Experiment.Conclusion Autologous cartilage combined with PRP can effectively repair rabbit osteochondral defects and promote the integration of normal surrounding cartilage.
