个体化营养治疗对老年机械通气患者营养状况及预后的影响 被引量:5

Effects of individualized nutrition therapy on nutritional status and prognosis in elderly patients with mechanical ventilation

摘要 目的探讨个体化营养治疗对老年机械通气患者营养状况及预后的影响。方法回顾性分析2015年7月至2017年7月四川省第四人民医院重症医学科收治的需机械通气的老龄危重患者96例,按营养方案的实施分为个体化营养支持组和传统营养支持组,对两组患者营养支持前、第2周、第4周各项营养指标、机械通气时间、抗感染治疗时间、住院费用、转归进行比较分析。结果4周后,个体化治疗组患者与治疗前比较血清总蛋白、白蛋白、前白蛋白、血红蛋白水平明显上升(P<0.05),传统治疗组患者也较治疗前有所上升(P<0.05),但个体化治疗组与传统治疗组相比上升更明显(P<0.05)。个体化治疗组在机械通气时间、抗感染治疗时间及60d死亡率较传统治疗组明显降低(P<0.05),但两组患者30d住院费用相比差异无统计学意义(P=0.05)。结论在不增加患者经济负担的情况下,个体化营养支持能够明显改善住院患者的营养状态,缩短患者机械通气时间,改善患者的预后。 Objective To explore the effect of individualized nutrition therapy on the nutritional status and prognosis of elderly patients with mechanical ventilation .Methods Ninety six aged critically ill patients with mechanical ventilation in the Fourth People′s Hospital of Sichuan Province from July 2015 to July 2017 were retrospectively analyzed .It is divided into individual nutrition support group and traditional nutrition support group according to the implementation of the nutrition program . The nutritional indicators ,mechanical ventilation time ,anti infection treatment time ,hospitalization expenses and outcome of two groups of patients before ,second and fourth weeks before and after nutritional support were compared .Results After 4 weeks ,individual patients in the treatment group compared with before treatment ,serum total protein ,albumin ,prealbumin ,hemoglobin levels were significantly increased ( P < 0 .05) ,conventional treatment group patients than before treatment increased ( P < 0 .05 ) ;individual treatment group compared with the conventional treatment group increased more obviously ( P < 0 .05) . In individualized treatment group ,mechanical ventilation time ,anti infection treatment time and 60 days mortality rate were significantly lower than those in traditional treatment group ( P < 0 .05) ,but there was no significant difference in hospitalization cost in 30 days ( P = 0 .05 ) .Conclusions without increasing the economic burden of patients ,individualized nutritional support can significantly improve the nutritional status of hospitalized patients ,shorten the time of mechanical ventilation and improve the prognosis of patients .
