摘要
目的 通过对老年住院患者营养状况与营养干预情况的研究,分析营养干预与临床预后的关系。方法 采用微型营养评定简表(the miniature nutritional assessment-short form,MNA-SF)对2020年8月—2021年7月于山东大学齐鲁医院住院且符合纳入标准的患者进行营养风险筛查以及营养状况评估,并获取住院期间的营养干预情况,统计分析营养干预后患者的临床预后情况。结果 共422例患者纳入研究,其中156例(36.9%)患者存在营养风险(营养不良+营养不良风险),其发生率随年龄、疾病种类数的增长而增加。存在营养不良的患者均接受了营养干预(50/50),营养正常的患者均未接受营养干预(0/266)。接受营养干预的患者中,肠内营养(enteral nutrition,EN)为主要的干预方式,EN、肠外营养(parenteral nutrition,PN)、联合营养(EN+PN)应用比为1.68∶1.00∶1.09。67例患者入院1周后接受了第2次营养风险评估,42例患者2周后接受了第3次营养风险评估,评估结果显示,患者接受营养干预后营养指标(前白蛋白、血红蛋白、淋巴细胞计数)均有显著的改善,差异有统计学意义(P <0.05)。结论 老年住院患者营养风险发生率较高,合理的营养干预对老年患者营养状态的改善有显著作用,个体化、细致化的选择合适的营养干预方式极为重要。
Objective:To analyze the relationship between nutritional intervention and clinical prognosis by studying the nutritional status and nutritional intervention in elderly hospitalized patients.Methods:The miniature nutritional assessment-short form(MNA-SF)was used to assess the nutritional risk and nutritional status of patients who were admitted to a Grade-A tertiary hospital in Shandong Province(hereinafter referred to as our department)from August 2020 to July 2021 and met the inclusion criteria,and to obtain the nutritional interventions during hospitalization,and to statistically analyze the clinical prognosis of the patients after nutritional interventions.Results:422 patients were included in the study,of whom 156 patients were at nutritional risk(malnutrition+risk of malnutrition)with a prevalence of 36.9%,and the prevalence increases with age and number of disease types.All patients with malnutrition received nutritional intervention(50/50)and none of the patients with normal nutrition received nutritional intervention(0/266).Among the patients who received nutritional intervention,enteral nutrition(EN)was the main mode of intervention,and the application ratio of EN,parenteral nutrition(PN),and combined nutrition(EN+PN)was 1.68∶1.00∶1.09.67 patients received the 2nd nutritional risk assessment after 1 week of admission,and 42 patients received the 3rd nutritional risk assessment after 2 weeks.The results of the 2 assessments showed that the patients who received nutritional intervention had significant improvement in nutritional indicators(prealbumin,hemoglobin,and lymphocyte count).Conclusion:Elderly hospitalized patients have a high incidence of nutritional risk,and rational nutritional intervention has a significant effect on the improvement of nutritional status of elderly patients,and it is extremely important to select appropriate nutritional interventions in an individualized and detailed way.
