摘要
目的为微钛板种植体辅助上颌前方牵引的临床应用提供参考。方法建立两类(L/Y)改良型微钛板种植体辅助上颌前方牵引的三维有限元模型,模拟牵引力大小为4.9 N/侧,牵引方向与上颌牙合平面呈0°~50°斜向前下,分析各工况下上颌骨的位移趋势和变化规律。结果 L、Y型微钛板种植体辅助上颌前方牵引对上颌骨位移趋势的影响基本一致;上颌前方牵引时,上颌牙列有缩窄的趋势;牵引方向斜向前下与牙合平面呈在0°~20°时,上颌骨发生逆时针旋转,30°~50°时,上颌骨发生顺时针旋转。结论 (1)微钛板种植体的形态对前方牵引时上颌骨位移趋势影响较小;(2)上颌前方牵引时,上牙列有缩窄趋势;(3)上颌骨位移趋势会随着牵引方向的改变而不同,牵引方向斜向前下与牙合平面呈0°~20°时,对治疗反覆牙合患者有积极意义,30°~50°时,对治疗反牙合伴开牙合趋势的患者有积极意义。
Objective To provide reference for clinical application of miniplate implants assisting maxillary complex for maxillary protraction treatment.Methods The three-dimensional finite element models of the maxillary complex with two types (L/Y)of modified miniplate implants were established.4.9N/side was applied to each modified miniplate implant respectively,the direction was forward and downward between 0°-50°degrees to the occlusion plane.The trend of maxillary displacement was analyzed.Results The application of two types(L/Y)of modified miniplate implants to assist the maxillary protraction showed that effects on the displacement of the maxilla were basically consistent.In the application of modified miniplate implants to assist the maxillary protraction treatment,the maxillary dentition had a tendency to narrow.When pulling the maxilla forward and downward,and the angle to the occlusal plane was 0°-20°,the maxilla rotated counterclockwise.When the angle was 30°-50°,the maxilla rotated clockwise.Conclusion ②The morphology of the modified microtitan plate implants do not affect the displacement of the maxilla in the maxillary protraction treatment.①The maxillary dentition becomes narrower after maxillary protraction treatment.③The displacement trend of maxilla varies with the direction of traction.When the traction direction is forward and downward at the angle of 0°-20°,there is a counterclockwise rotation in the maxilla,which is beneficial to deep crossbite patients.When at 30°-50°,there is a clockwise rotation in the maxilla,which is beneficial to crossbite patients accompanied by open bite.
