摘要
目的探讨微创牙体调磨改善上颌前牙美观的临床效果。方法选取2013年6月至2016年10月解放军第四一一医院收治的上颌前牙轻度牙体形态异常而影响美观的患者58例,采用微创牙体调磨方法对255颗上颌前牙进行了调磨抛光,调磨范围主要包括牙齿切端、近远中切角、唇侧轴面角等。调磨前后采用白色美学指数评分(white esthetics score,WES)进行疗效评价,调磨后即刻、3个月后采用视觉模拟评分法(visual analogue scale,VAS)分别对患者进行美观改善程度分析比较。结果所有患者前牙美观程度在调磨后均得到了不同程度的改善。所有患者美观改善程度VAS评分治疗3个月后(8. 12±1. 06)分较调磨后即刻(7. 90±1. 20)分增高不明显,差异无统计学意义(P> 0. 05)。所有患者调磨后牙齿的WES评分为(8. 88±0. 78)分较调磨前(5. 83±1. 03)分明显提高,差异有统计学意义(P <0. 05)。3个月随访期内调磨牙齿无持续过敏、疼痛等。结论微创牙体调磨能在不影响牙体牙髓健康的前提下,有效改善部分患者上颌前牙的美观,值得临床推广。
Objective To investigate the clinical effect of minimally invasive dental grinding on the white esthetics scores in maxillary anterior teeth.Methods Fifty-eight patients who received dental treatment in the hospital from June 2013 to October 2016 due to mild abnormal form were enrolled for the study.Minimally invasive dental grinding was used to grind and polish the affected maxillary anterior teeth.The grinding areas included cut end,cut point and axial area of the tooth.Before and after grinding,the white esthetics scores (WES)were used to evaluate treatment effects,and improvement of aesthetics was evaluated and compared by visual analogue scale (VAS),the moment before grinding and 3 months after grinding.Results Aesthetics of the maxillary anterior teeth in all the patients improved to a certain extent.No significant increase in VAS scores 3months after treatment (8.12±1.06)could be seen in all the patients;as compared with the scores obtained the moment after treatment(7.90±1.20),and no statistical ,significance could be found when comparisons were made between them (P >0.05).The WES scores of all the patients after grinding were (8.88±0.78),which were significantly higher than those before grinding (5.83±1.03),with statistical significance (P <0.05).Threemonth medical follow-ups indicated that there were no persistent allergy,pain and other abnormalities.Conclusion Minimally invasive grinding could effectively improve aesthetics of maxillary anterior teeth in certain patients on the premise that little harm would be exerted on the health of dental pulp.For this reason,it was worth further clinical popularization.
