摘要
目的 :探讨产ESBLs(超广谱β-内酰胺酶)大肠埃希菌的临床分布特征及耐药性。方法 :对2016年1月至2018年1月在启东市中医院各科室收集的198株大肠埃希菌菌株进行分离和鉴定,并对被检出的产ESBLs大肠埃希菌菌株进行耐药试验。然后,观察这些大肠埃希菌菌株中产ESBLs大肠埃希菌菌株的阳性检出率,观察被检出的产ESBLs大肠埃希菌菌株的标本类型和标本来源及其对各类抗菌药的耐药性。结果 :在这198株大肠埃希菌菌株中,其产ESBLs菌株的阳性检出率为46.5%。在这92株产ESBLs大肠埃希菌菌株中,在送检的痰液标本中的产ESBLs大肠埃希菌菌株的检出率最高,在送检的重症监护室的标本中产ESBLs大肠埃希菌菌株的检出率最高。这92株产ESBLs大肠埃希菌菌株对青霉素类药物、喹诺酮类药物、三代头孢菌素类药物等药物均表现出较强的耐药性。亚胺培南类抗菌药对产ESBLs大肠埃希菌具有极高的抗菌活性。结论 :痰液标本是产ESBLs大肠埃希菌的主要标本来源,重症监护室最易产生产ESBLs大肠埃希菌。临床上在治疗由产ESBLs大肠埃希菌所致感染性疾病时,应为患者首选亚胺培南进行治疗。
objective: to investigate the clinical distribution characteristics and drug resistance of esbls-producing e. coli. Methods: from January 2016 to January 2018, 198 strains of escherichia coli were isolated and identified in each department of qidong hospital of traditional Chinese medicine. Then, the positive detection rate of middle ESBLs escherichia coli strain was observed, and the type and source of the tested ESBLs ESBLs escherichia coli strain and its resistance to various antibiotics were observed. Results: among the 198 strains of ESBLs, the positive detection rate was 46.5%. Among the 92 esbls-producing ESBLs escherichia coli strains, the ESBLs ESBLs escherichia coli strains in the sputum specimens submitted for examination had the highest detection rate, and the ESBLs ESBLs escherichia coli strains were the most detected in the specimens submitted for examination in the intensive care unit. These 92 strains of ESBLs escherichia coli strains showed strong resistance to penicillin, quinolone and third-generation cephalosporins. Imipenem has high antibacterial activity to ESBLs escherichia coli. Conclusion: sputum specimens are the main source of ESBLs escherichia coli. In clinical treatment of infectious diseases caused by ESBLs escherichia coli, imipenem should be the first choice for patients.
