摘要
为探究股骨远端微创内固定系统(LISS)对治疗股骨粗隆间粉碎性骨折的临床恢复效果,本研究选取哈励逊国际和平医院既往手术治疗的126例股骨粗隆间骨折患者,对每例股骨粗隆间骨折患者的手术数据进行回顾性分析。样本收集时间区间为2014年1月至2016年9月,根据骨折手术方法的不同分别分为LISS组(采用LISS系统内固定治疗68例)、DHS组(采用动力髋螺钉(DHS)内固定治疗58例),对比两组患者的手术情况、术后髋关节功能的恢复差异。研究结果显示,LISS组的手术时间、术中出血量、切口长度均显著的低于DHS组,差异具有统计学意义(p<0.05);LISS组患者与DHS组的骨折平均愈合时间差异无统计学意义(p>0.05);术后3个月、6个月,LISS组的Harris功能评分均高于DHS组,差异具有统计学意义(p<0.05);术后6个月,LISS组的髋关节功能优于DHS组,差异具有统计学意义(p<0.05);LISS组与DHS组的并发症率分别为2.94%和6.90%,两者相比较,差异不具有统计学意义(p>0.05)。研究结果表明,对比DHS手术方法,LISS治疗股骨粗隆间粉碎性骨折具有创伤小、恢复快、术后髋关节功能恢复良好的特点。
To investigate the clinical recovery effect of distal femoral less invasive internal stabilization system (LISS) in the treatment of intertrochanteric comminuted fracture, this study selected 126 cases of intertrochanteric fractures treated by Harrison International Peace Hospital in the past to retrospectively analyze the operative data of every patient with intertrochanteric fracture. Samples were collected from January 2014 to September 2016 and were divided into LISS group (68 cases treated with LISS system internal fixation) and DHS group (58 cases underwent dynamic hip screw (DHS) fixation) according to surgical methods.. The operative conditions and postoperative recovery of hip joint function between the two groups were compared. The results showed that the operative time, intraoperative blood loss and incision length in LISS group were significantly lower than those in DHS group, the difference was statistically significant (p<0.05); The mean fracture healing time in LISS group was not significantly different from DHS group (p<0.05); 3 months, 6 months after surgery, the scores ofHarris function in LISS group were higher than those in DHS group, the difference was statistically significant (p<0.05). 6 months after operation, the hip joint function of LISS group was better than that of DHS group, the difference was statistically significant (p<0.05); The complication rate in LISS groupwas 2.94%comparedwith 6.90%inDHS group, the differencewas not statistically significant (p<0.05). The results showed that compared with DHS method, LISS treatment for comminuted intertrochanteric fracture had the characteristics of small trauma, quick recovery and good recovery of postoperative hip function.
