股骨近端钢板内固定治疗股骨粗隆间骨折手术并发症分析 被引量:8

Analysis of complications of femoral intertrochanteric fracture treated with proximal femur plate

摘要 目的分析股骨近端钢板内固定治疗股骨粗隆间骨折手术并发症的发生原因并探讨减少并发症的方法。方法总结自2009-01—2011-12采用股骨近端解剖钢板及股骨近端锁定钢板内固定治疗的56例股骨粗隆间骨折,对8例出现手术并发症的原因进行了分析。结果 8例出现内固定松动,6例出现髋内翻,2例骨折复位不良,3例异位骨化。内固定选择不当、内固定设计缺陷及使用技巧不足、股骨粗隆部内后侧(压力侧)小粗隆及股骨距支撑力不足及外上侧(张力侧)大粗隆骨折未固定是发生手术并发症的主要原因。结论根据骨折类型、内固定特点及患者自身状况选用内固定材料,改进内固定设计及使用技巧,注重压力侧及张力侧骨折块的复位固定是减少发生手术并发症的主要方法。 Objeetive To analyze the complications of femoral intertroehanterie fracture which were treated with proximal femur plate and to find the ways of decreasing the complications. Methods Fifty-six patients with femoral intertroehanteric fracture were treated with proximal femur plate from January 2009 to December 2011. Among them 8 eases showed different complications and the eomplications were analyzed. Results The eomplications included screw loosening(8 cases), coxa vara(6 cases), incomplete fracture reduction(2 eases) and heterotopic ossifieation(3 eases). The main causes of the complications were wrong selection of the plate for operation,defect of plate design,instffficient applying skill,insufficient support of calcar femorale and untreated fracture of greater trochanter. Conclusion The proper selection and using of the plate,improvement of plate designing and excellent fixation of ealcar femorale and greater trochanter are the main ways to decrease the complications.
