摘要
目的研讨全髋关节置换术患者采取改良外侧小切口入路的临床价值。方法选择2016年7月至2018年7月在我院行全髋关节置换术的58例患者参与实验观察,同时随机分配患者入组,Ⅰ组(n=29)采取标准后外侧Gibson入路,Ⅱ组(n=29)经改良外侧小切口入路处理,分析评估两组手术入路的应用情况。结果两组在切口长度方面的对比,有统计学意义(P<0.05),但手术时间、术中出血量对比,无统计学意义(P> 0.05)。Ⅰ、Ⅱ组术后6周的髋臼外展角依次是(43.8±4.52)°和(44.3±4.13)°,对比无统计学意义(P> 0.05),髋臼前倾角依次是(18.8±5.14)°和(13.3±3.88)°,对比有统计学意义(P<0.05)。Ⅱ组术后6周的Harris与VAS评测结果均显著优于Ⅰ组,比较有统计学意义(P<0.05),术后12周、24周Harris与VAS评测结果的对比,无统计学意义(P> 0.05)。结论经改良外侧小切口入路对患者行全髋关节置换术治疗,切口较小、疼痛缓解较快,并可有效促进髋关节功能的恢复,值得推荐。
Objective To investigate the clinical value of modified lateral small incision approach in patients undergoing total hip arthroplasty. Methods From July 2016 to July 2018, 58 patients undergoing total hip arthroplasty in our hospital were involved in the experimental observation. Patients were randomLy assigned to receive the disease. Group Ⅰ(n=29) received a standard posterolateral Gibson approach. Group Ⅱ(n=29) was treated with a modified lateral small incision approach, and the application of the surgical approach was evaluated. Results There was a statistically significant difference in the length of the incision between the two groups(P< 0.05), but there was no statistically significant difference in the operation time and intraoperative blood loss(P>0.05). The acetabular abduction angle of group Ⅰ and group Ⅱ was(43.8±4.52)° and(44.3±4.13)° at 6 weeks after operation. The comparison was not statistically significant(P> 0.05), and the acetabular anteversion angle was(18.8±5.14)° and(13.3±3.88)°, the comparison was statistically significant(P< 0.05). The results of Harris and VAS in group Ⅱ were significantly better than those in group Ⅰ at 6 weeks after operation(P<0.05). There was no statistical significance between Harris and VAS in 12 weeks and 24 weeks after surgery(P>0.05). Conclusion The modified small lateral incision approach is used for total hip arthroplasty. The incision is small, the pain is relieved quickly, and the recovery of hip function can be effectively promoted. It is recommended.
