摘要
目的探究人工关节置换术与股骨近端髓内钉治疗老年股骨粗隆间骨折的临床疗效及对髋关节功能的影响。方法选取我院2016年8月~2017年8月收治的老年股骨粗隆间骨折患者128例,按随机数字表法分为两组,各64例。对照组以人工关节置换术治疗,观察组以股骨近端髓内钉治疗。对比两组临床疗效及髋关节功能变化。结果观察组术中出血量少于对照组,住院时间及卧床时间均短于对照组,差异显著,有统计学意义(P<0.01);观察组术后站立时间长于对照组,差异有统计学意义(P<0.01);手术时间与对照组相比无明显差异(P>0.05);观察组术后1个月、3个月、6个月人工全髋关节疗效评分表(Harris)评分均高于对照组,差异显著,有统计学意义(P<0.01)。结论髓内钉固定治疗老年股骨粗隆间骨折较人工关节置换术可进一步提高临床疗效,促进患者术后髋关节功能恢复,值得临床推广。
Objective To explore the efficacy of artificial joint replacement and proximal femoral nail in the treatment of in-tertrochanteric fracture and the effect on the hip function in the elderly. Methods A total of 128 elderly patients with intertrochanteric fractures treated in our hospital from August 2016 to August 2017 were randomly divided into two groups. The control group (64 ca-ses) was treated with artificial joint replacement. The observation group (64 cases) was treated with proximal femoral nail. The clini-cal efficacy and hip function were compared between the two groups. Results In the observation group, the amount of bleeding was less and the length of hospital stay and bed time were shorter than those in the control group (P<0. 01). The postoperative standing time in the observation group was longer than that in the control group ( P<0. 01). There was no significant difference in operation time between two groups (P>0. 05). The Harris scores at 1 month, 3 months, and 6 months after surgical operation were higher than those in the control group (P<0. 01). Conclusion Intramedullary nailing for the treatment of intertrochanteric fractures of the femur in the elderly can further improve the clinical efficacy than artificial joint replacement and promote the recovery of hip function af-ter surgery.
