The treatment of diabetic macular edema with multi-wavelength laser

摘要 探讨多波长激光治疗糖尿病性视网病变的临床疗效。方法 :对 5 0例 (80眼 )患有糖尿病性视网膜病变和黄斑水肿的 型糖尿病患者进行多波长激光治疗 ,其中非增殖期糖尿病性视网膜病变 NPDR2 5例 42眼 ,增殖期糖尿病视网膜病变 PDR2 5例 38眼。治疗中 ,黄斑区治疗 ,以黄光治疗为主 ,包括局灶性光凝和弥漫性格栅样光凝。周边光凝则以黄绿混合光、绿光或红光进行治疗。在一些复杂的病例治疗中 ,则采用几种波长激光随时切换使用 ,达到以最低能量最低损害 ,产生有效光斑和最大治疗效果的目的。结果 :本组病例治疗前视力均值为 0 .36± 0 .2 9,光凝后的视力均值为 0 .47± 0 .31,P<0 .0 5黄斑水肿消减率达 71.2 5 %。NPDR的视力提高或不变值为 92 .85 % ,PDR者为 76 .31% ,P<0 .0 5。结论 :多波长激光治疗视网膜病变有效 ,且早期治疗效果好 ,氪黄激光治疗糖尿病性黄斑水肿疗效较好。 Objective: To discuss the clinic sense of multi-wavelength laser on the treatment of diabetic macular edema (DME). Methods: 50 NIDDM patients with diabetic retinopathy and macular edema were treated with multiple wavelength lasers. Among them, Twenty-five were NPDR (non-proliferative diabetic retinopathy), with forty-two eyes; and twenty-five were PDR (proliferative diabetic retinopathy) with thirty-eight eyes. The average visual acuity before treatment was 0.36±0.29Yellow laser was used for treating the macular edema including direct photocoagulation and vague photocoagulation. For the pan-retinal area, yellow and green, green or red laser was used. In some complicated patients, the best laser wavelength chosen, the best result obtained. Results: After treatment, the average visual acuity was 0.47±0.31, P<0.05. The macular edema reduced in 71.25% of the patients. The vision increased or no change in the NPDR patients was 92.85%, and in PDR was 76.31%, P<0.05. Conclusions: The better result would be obtained at the early period during the treatment of diabetic macular edema with multiple wavelength lasers. The Yellow laser seems the best wavelength for macular edema.
