摘要
目的 研究年轻心肌梗死患者 (首次发病年龄≤ 45岁 )纤维蛋白原基因 βG 455 A单碱基置换 ,探讨遗传因素对年轻心肌梗死患者早期发病的影响。方法 选择 1 998年 1月至 1 999年 1月住北京阜外医院行冠状动脉造影检查的男性心肌梗死患者 ,按首次发病年龄分组 ,≤ 45岁首次发病者 75例 (≤ 45岁组 ) ,≥ 46岁首次发病 97例 (≥ 46岁组 ) ,健康对照组 43例。采用聚合酶链反应(PCR)及寡聚核苷酸 (ASO)探针杂交技术检测纤维蛋白原基因 βG 455 A单碱基置换 ,同时采用组织凝血活酶法测定血浆纤维蛋白原浓度 (mg/dl)。结果 (1 )≤ 45岁组与≥ 46岁组和健康对照组相比A/A ,G/A基因型频率及A等位基因频率明显增高 (P <0 0 0 1 )。 (2 )在 3组内 ,A/A ,G/A基因型血浆纤维蛋白原水平明显高于G/G基因型 ,而且存在明显正相关 (r=0 49,P <0 0 0 1 )。心肌梗死患者血浆纤维蛋白原浓度明显高于健康对照组 (P <0 0 5)。在心肌梗死患者中 ,≤ 45岁组与≥ 46岁组相比血浆纤维蛋白原浓度相对较低 ,分别为 (348 7± 93 8)mg/dl和 (355 1± 95 7)mg/dl,尽管两组差异无统计学意义 (P >0 0 5) ,但分析冠心病其他危险因素可以进一步说明血浆纤维蛋白原浓度不仅受遗传因素的影响 ,亦受年龄、吸烟因素的影响。年龄增大 ,吸?
Objective To study G 455 A polymorphism of β fibrinogen gene in young patients with myocardial infarction (≤45 years old )and to explore the effect of inherited factor on young patients with myocardial infarction Methods G 455 A polymorphism of β fibrinogen gene was analyzed by PCR ASO hybridization in 75 young male (≤45 years old )and 97 old male (≥46 years old )patients with first myocardial infarction and 43 healthy controls The plasma fibrinogen levels were assayed by tissue thromboplastin activity (mg/dl) Results (1) The frequencies of the genotypes A/A, G/A and allele A were significantly higher in the younger group (≤45 years old )than those in the older group (≥46 years old ) and healthy group ( P <0 001); (2) In the three groups, plasma fibrinogen levels were greatly increased in individuals with A/A and G/A genotypes, compared with those in individuals with G/G genotype There was obviously positive correlation between A/A and G/A genotype and fibrinogen levels ( r =0 49, P <0 001) Plasma fibrinogen levels were significantly higher in patients with myocardial infarction than those in healthy group ( P <0 05) Plasma fibrinogen levels were relatively lower in ≤45 year old patients than those in ≥46 year old patients [(348 7±93 8) mg/dl vs (355 1±95 7) mg/dl], but there was no significant difference between the two groups ( P >0 05) It further showed that plasma fibrinogen levels were affected by not only inheritance but also environmental factors, for example, age and smoking Plasma fibrinogen levels were relatively increased with age and smoking index Conclusion The polymorphism frequencies of fibrinogen β G 455 A gene are higher in young patients with myocardial infarction It might be one of the possible inherited factors for early coronary heart disease
