摘要
目的 研究白介素 17(IL 17)在溃疡性结肠炎 (UC)的表达和分泌及与疾病活动性的关系。方法 用ELISA法测定UC患者及正常对照者血清或细胞培养液中 ,IL 17、IL 6和IL 8的浓度 ,逆转录聚合酶链反应 (RT PCT)测定IL 17mRNA的表达。结果 32例UC患者外周血中IL 17,IL 6和IL 8的浓度与 40例正常对照者比较 ,差异无显著性 (P >0 .0 5 ) ,但外周血CD+4T细胞在PMA和抗CD3 的刺激下 ,表达IL 17mRNA及分泌IL 17的水平均明显高于对照组 [(2 3.6± 5 .7) pg/ml和 (13.1± 3.2 ) pg/ml,P <0 .0 1]。UC患者病变部位的黏膜固有层CD+4T细胞 (LP CD+4T)与非受累部位的LP CD+4T细胞比较 ,它们表达大量的IL 17mRNA并自发分泌大量的IL 17蛋白 ,且IL 17浓度与该部位的单个核细胞 (LPMC)分泌的IL 6 ,以及患者外周血中的C 反应蛋白 ,血沉均呈显著正相关。在刺激剂的作用下 ,病变部位的LP CD+4T细胞IL 17的分泌进一步增加 ,且明显高于非受累部位LP CD+4T细胞的分泌水平。另外 ,UC病变部位LPMC分泌的IL 6和IL 8的水平均明显高于非受累部位的LPMC ,但在培养液中加入抗IL 17单克隆抗体后 ,LPMC细胞IL 6和IL 8的分泌均明显被抑制。结论 UC患者病变部位的LP CD+4T细胞表达和分泌IL 17明显增加 ,并与疾病的活动性呈正相关。抗IL
Objective To investigate the expression an d the role of interleukin 17(IL 17) in patients with ulcerative colitis (UC). Methods IL 17,IL 6 and IL 8 were measured using ELISA technique , and IL 17 mRNA was assayed by RT PCR in 32 patients with UC and compared with 40 controls. The effect of anti IL 17 m onoclonal antibody (MoAb) on production of IL 6 and IL 8 by lamina propria mo nonuclear cells (LPMC) was studied. Results As compared with controls, serum concentrations of IL 17 ,IL 6 and IL 8 from UC patients were significantly higher, but with no statist ic difference. The expression of IL 17 mRNA and secretion of IL 17 by the peripheral blood CD + 4 T cells in UC patients were higher than that in nor mal controls in the presence of stimuli (both P <0.01). Lamina propria CD + 4 T cells (LP CD + 4 T) from the involved colons of 23 patient s spontaneously produced high level of IL 17, which significantly correlated wi th the serum CRP levels, ESR and IL 6 concentration produced by LPMC from invol ved colon. The expression of IL 17 mRNA of LP CD + 4 T ce lls from the involved colons was detected in 22 out of 23 patients, but only 6 o ut of 23 LP CD + 4 T cells from uninvolved colons expressing IL 17 mRNA ( P <0.01). Furthermore, in the presence of the stimuli, the level of IL 17 produced by LP CD + 4 T cells from involved colons was significantly hig h er than that from uninvolved colons. IL 6 and IL 8 levels produced by LPMC fro m involved colons were significantly higher than that from uninvolved colons. Ad ding exogenous anti IL 17 MoAb to LPMC from involved colons resulted in dec rease in IL 6 and IL 8 production. Conclusions IL 17 was found to be produced significantly by LP CD + 4 T from involved colons, and was correlated with disease activity. Anti IL 17 MoAb inhibited the IL 6 and IL 8 secretion by LPMC.
