摘要
目的 经胸超声心动图 (TTE)指导下置人 Am-platzer封堵器评价治疗成人继发孔型房间隔缺损 (ASD)的可行性和治疗效果 .方法 全组 74例 ,年龄 1 4~ 65 (33.6±1 4 .4)岁 .按照术中超声检测的不同 ,分为 TEE组 :36例 ,在TEE和 X线透视下监测封堵 ASD;TTE组 :38例 ,在 TTE和 X线透视下监测封堵 ASD.结果 两组患者 ASD的 TTE测量值、BSD值、封堵器直径和透视时间之间等均无显著性差异 (P>0 .0 5) .TTE组手术时间 (64.4± 1 3.6) min明显短于TEE组的 (76.1± 1 6.8) min(P<0 .0 1 ) .两组植入封堵器均获成功 ,TTE组有 1例因 ASD巨大、边缘较薄较短 ,改用TEE监测 .术中未发生任何重要并发症 ,无急诊手术病例 .术后完全闭合率无显著性差异 (P>0 .0 5) .随访 1~ 3 mo,封堵器形态 ,位置良好 ,未对毗邻结构产生影响 .结论 TTE指导下经导管置入 Amplatzer封堵器治疗 ASD是一种安全、有效。
AIM To evaluate the treatment efficiency and the feasibility of transcather closure of secundum atrial septal defect (ASD) using Amplatzer septal occluder (ASO) device with transthoracic echocardiography (TTE) in adults. METHODS There were 74 patients. The mean age was (33.6±14.4) years (ranged from 14 to 65 years). The patients were divided into TEE group and TTE group according to the guidance used in ASO closure. There were 36 ASD patients whose closure procedure was guided by fluoroscopy and TEE in TEE group. There were 38 patients by fluoroscopy and TTE in TTE group. RESULTS There was no significant difference between the two groups in ASD diameter measured by TTE and BSD, the size of implanted ASO devices and fluoroscopy time ( P >0.05). The procedural time in TTE group was shorter significantly than that in TEE group [(64.4±13.6) min vs (76.1±16.8)min; P <0.01]. There were all successful implants in both groups. One case in TTE group was done successfully by adding transesophageal echocardiography for guidance in the procedure. The rates of complete closure of ASD were not significantly different between the two groups soon after implantation, 24 h after implantation, and 1~3 mo follow up duration ( P >0.05). And all patients had no displacement and recanalization at 1~3 mo follow up. CONCLUSION Transcatheter closure of secundum ASD occlusion using ASO with TTE guidance is an efficient, feasible and simple method in adults.
