摘要
目的克服在胆道损伤修复重建术中丝线缝合加重瘢痕形成并使吻合口狭窄之不足。方法用合成粘合剂(OB 胶)替代缝线,结合内支撑导管建立了各种胆肠吻合口瘢痕动物实验模型,并定期对吻合口瘢痕组织进行病理形态学观察和计算机图像处理(ECVD),从瘢痕形成的角度观察胆肠吻合口愈合过程。结果 OB 胶可促进吻合口愈合,减少瘢痕形成,加速瘢痕成熟软化;内支撑保留6个月瘢痕成熟。结论 OB 胶粘合是预防胆道损伤修复重建术后吻合口狭窄的一种有效措施。
Objective To overcome the drawbacks of conventional suture technique in bile duct reconstruction usually resulting in anastomotic stenosis as well as eventual obstructive jaundice and bil- iary cirrhosis.Methods OB glue was used instead in the formation of biliointestinal anastomosis where indwelling stent was inserted to complete the experimental model.Pathological study and extra- cellular collagen volume density (ECVD) evaluation of the anastomotic specimens were conducted in order to follow up the whole process of anastomotic wound healing.Results The OB glue promoted wound healing,limited scar formation and accelerated scar softening and maturation.OB glue plus 6- month indwelling stent could ensure scar maturation.Conclusions OB glue conglutinating biliointesti- nal anastomosis is an effective method for prevention of bile duct stricture.
