摘要
目的 探讨 p16、p2 1蛋白和增殖细胞核抗原 (PCNA)在胃癌发生、发展中的作用及临床意义。方法 应用免疫组织化学法 ,对 46例胃癌及癌旁组织中p16、p2 1蛋白和PCNA进行检测。结果 胃癌组织中 p16、p2 1蛋白和PCNA的阳性率分别为 2 1.0 5 %、2 8.2 6%、73.91% ,与癌旁组织中的阳性率 5 8.70 %、65 .2 1%、2 3.91%差异有显著性 (P <0 .0 5 )。在胃癌的不同病理参数(淋巴结转移 ,分化程度 ,浸润深度及TNM分期 )中 ,p16,PCNA和 p2 1蛋白表达则分别在全部 4个 ,前 3个 ,前 2个参数中存在明显差异 (P <0 .0 5 )。p16、p2 1蛋白和PCNA在肿瘤的部位、大小、患者性别及年龄间差异无显著性 (P >0 .0 5 )。p16原癌基因蛋白质与PCNA ,p2 1与PCNA阳性表达存在明显差异 (P <0 .0 5 ) ,而p2 1表达与 p16表达之间则差异无显著性 ( P <0 .0 5 )。结论 p16、p2 1蛋白和PCNA是胃癌发生、发展过程中的重要因素 ,且与胃癌临床病理参数密切相关。
Objective To study the clinical implicati on and effects of the expression of p16,p21 protein as well as PCNA in the occu rrence and development of gastric carcinoma.Methods p16, p21 protein and PCNA expression was detected in 46 specimens of gastric carcinom a and pericarcinoma tissues by the methods of immunohistochemistry. R esults The positive rate of p16, p21 protein expression (21.05%, 28 .26%) in gastric carcinoma samples were significantly lower than in pericarcinom a tissues (58.70%, 65.21%) respectively. The positive rate of PCNA (73.91%) in g astric carcinoma samples was significantly higher than in pericarcinoma tissues (23.91%). In different clinical parameters of the gastric cancer (lymph node met astasis, degree of differentiation, depth of invasion and clinical stage), the e xpression of p16 protein, PCNA and p21 protein was significantly different in th e all four, the former three, the former two parameters respectively.The express ion of PCNA was significantly different between the positive expression of p16, p21 protein and the negative expression of them. There were no different between the express ion of p16 protein and p21 protein. The expression of p16, p21 protein and PCNA was not correlated with sex, age, tumor size and sites (P>0.05). Conclusion The abnormal expression of p16, p21 protein and PCNA mi ght play a great role in the occurrence and development of gastric carcinoma, an d be closely correlated with the clinical pathological parameters of gastric car cinoma.
