摘要 目的 研究细胞增殖状态与肺癌临床病理生理特征和预后的关系 ,探讨 Ki6 7对临床诊断和预后判断的价值。 方法 应用免疫组织化学法检测 16 6例肺癌组织标本和 37例肺良性病变组织中 Ki6 7的表达水平。 结果 (1)肺癌组织中 Ki6 7指数显著高于肺良性病变组织 (P<0 .0 1) ,而癌旁组织与良性病变组织相比差别无显著性意义 ;(2 )肺癌组织中 Ki6 7表达水平增强与肺癌组织学类型、细胞分化程度有关 (P<0 .0 5 ) ,而与患者 PTNM分期、淋巴结转移程度、吸烟与否以及患者的性别、年龄无明显关系 ;(3) Ki6 7高表达组肺癌患者的术后生存时间显著低于低表达组 (P<0 .0 1) ,按 PTNM分期分层后这种差别仍然存在 (P<0 .0 5 )。 结论  Ki6 7能较好地反映肺癌细胞的增殖状态 。 Objective To investigate the relationship of Ki67 with the clinicopathological parameters and prognosis of primary lung cancer patient. Methods The expression of Ki67 gene were detected in 166 lung cancer samples and 37 benign lesion tissues of lung as control by immunohistochemistry. Results (1) The Ki67 index was significantly higher in lung cancer tissues than in benign lesion tissues(P<0.01), otherwise, no notable difference was found between normal tissues adjacent to cancer and controls(P>0.05); (2) The expression level of Ki67 gene was closely related to histological classification and cancer cell differentiation(P<0.05), and there was no significant relationship between Ki67 expression and PTNM stages, lymph node involvement, smoking habit, sexuality and age of lung cancer patients (P>0.05); (3) The postoperative survival time of patients in high Ki67 expression group was significantly shorter than those in low expression group (P<0.01), and this reverse association between Ki67 index and survival time is independent to PTNM stages of patients(P<0.05). Conclusion Ki67 is a favorable marker to proliferation of lung cancer cells, and can provide useful information for diagnosis and prognosis of lung cancer patients.
