摘要
目的百令胶囊联合依地酸钠钙治疗成人慢性铅中毒肾损伤的临床效果。方法选择2011年6月—2013年2月我科治疗的39例成人慢性铅中毒患者,采用随机抽签法分为百令胶囊联合依地酸钠钙组(联合组,n=22)和依地酸钠钙组(n=17),检测两组患者治疗前后血铅、尿素氮、血肌酐、血尿酸、尿β2微球蛋白水平,并进行对比分析。结果两组患者的年龄、性别、铅接触时间、血铅、尿素氮、血肌酐、血尿酸及尿β2微球蛋白间差异均无统计学意义(P<0.05);联合组治愈率为82%(18/22),依地酸钠钙组治愈率为70.6%(12/17),差异无统计学意义(χ2=0.88,P=0.35)。组间比较,治疗后两组血铅、尿素氮水平间差异有统计学意义(P<0.05),两组血尿酸和尿β2微球蛋白水平间差异无统计学意义(P>0.05)。组内比较,治疗后依地酸钠钙组尿素氮、血肌酐及尿β2微球蛋白水平与治疗前比较,差异无统计学意义(P>0.05),血铅较治疗前降低(P>0.05),血尿酸较治疗前升高(P<0.05);联合组血铅、尿素氮、血肌酐及尿β2微球蛋白水平较治疗前下降(P<0.05),血尿酸与治疗前比较,差异无统计学意义(P>0.05)。结论对合并轻度肾损害的慢性铅中毒患者行依地酸钠钙针驱铅治疗安全有效,联合百令胶囊治疗可减轻慢性铅中毒肾损伤。
Objective To investigate the clinical effect of Corbin capsules combined with CaNa2 -EDTA on adult re- nal injury induced by chronic lead poisoning. Methods A total of 39 chronic lead poisoning patients treated in this hospital from June 2011 to February 2013 were divided randomly into groups A （treated with corbrin capsules combined with CaNa2 - EDTA, n =22）, B （treated with CaNa2 -EDTA, n = 17）. Levels of blood lead, bloodurea nitrogen （BUN）, serum creatinine （Cr）, blood uric acid （BUA）, B2 microglobulin （B2 -MG） were determined and analyzed comparatively in 2 groups before and ＇after treatment. Results There was no significant difference in age, gender, duration of lead exposure, blood lead, BUN, Cr, BUA, B2 - MG between 2 groups （P 〈0.05）. The cure rate was 82% （18/22） in group A, 70. 6% （12/17） in group B, the difference was not significant （X2 =0. 88, P =0. 35）. There was difference in blood lead, BUN between 2 groups after treatment （P 〈0.05）, there was not in BUA, B2 - MG （P 〉 0. 05）. No difference was noted in BUN, Cr, B2 - MG in group B after treatment （ P 〉 0. 05 ）, blood lead was lower, BUA was higher after treatment than before （ P 〈 0. 05 ） ; Blood lead, BUN, Cr, B2 - MG were lower after treatment than before in group A （ P 〈 0. 05 ） , and there was no difference in BUA between pre - and post - treatments （ P 〉 0.05 ）. Conclusion CaNa2 - EDTA is safe and effective for adult chronic lead poisoning with mild renal iniury, Corbrin capsules combined with CaNa2 -EDTA can alleviate renal injury induced by chronic lead poisoning.
