多层螺旋低张CTU与低张IVP合用在泌尿系统疾病诊断中的应用价值(附78例报告) 被引量:14

THE VALUE OF THE COMBINATION OF MULTILAYER SPIRAL HYPOTONIC CTU AND HYPOTONIC IVP IN THE DIAGNOSIS OF THE URINARY SYSTEM DISEASES

摘要 目的探讨腹部X线平片(KUB)加低张静脉尿路造影(IVP)与多层螺旋CT低张尿路造影合用对泌尿系统疾病诊断中的应用价值。方法对78例临床考虑泌尿系统疾病的患者做KUB平片加低张静脉尿路造影(IVP)的同时行多层螺旋CT低张尿路造影(CTU),并行3D尿路造影重建,将诊断结果与手术及病理结果对照。结果 78例中输尿管结石42例,输尿管结核4例,输尿管肿瘤9例,膀胱癌5例,输尿管炎性狭窄5例,先天畸形7例(包括肾输尿管连接部狭窄5例,肾输尿管重复畸形2例),腹腔或盆腔肿瘤及其他病灶压迫输尿管6例,KUB平片加低张静脉尿路造影(IVP)与多层螺旋CT低张尿路造影(CTU)合用,图像好,能充分显示肾实质、肾盂、输尿管、膀胱以及泌尿器官与周围组织的关系。结论KUB平片加低张静脉尿路造影(IVP)与多层螺旋CT低张尿路造影合用,对泌尿系统梗阻性病变有诊断价值,对炎症性病变鉴别诊断有帮助,尤其对肾功能严重受损的上尿路梗阻性疾病具有特殊的诊断价值。痛苦少、效果好,可作为临床泌尿系患者检查的首选方法。 Objective To discuss the value of the combination of KUB plain film with hypotonic IVP and multilayer spiral hypotonic CTU in the diagnosis of the urinary system diseases. Methods The 78 cases of urinary diseases were diagnosed by the clinical diagnosis with KUB plain film and hypotonic intravenous urography, 16 slice spiral CT hypotonic urinary tract imaging and 3D urography reconstruction, then com- pared the diagnosis results with the results of operation and pathology. Results In the 78 cases, there were 42 cases with ureteral calculi, 4 cases with tuberculosis of ureter, 9 cases with ureteral tumor, 5 cases with carcinoma of urinary bladder, 5 cases with inflammatory stricture of ureter, 7 cases with congenital malformation among with 5 cases with renal ureteropelvic junction stenosis and 2 cases with Ureteral duplication, and 6 cases with abdominal or pelvic tumors and other lesions oppression ureter. The combination of KUB plain film, hypotonic intravenous urography ( IVP ) and the multilayer spiral CT hypotonic urinary tract angiography could fully display the renal parenchyma, renal pelvis, ureter, bladder, urinary organs and tissue's relationship with the surrounding tissues by clear pictures. Conclusions The combination of KUB plain film, hypotonic intravenous urography ( IVP ) and the multilayer spiral CT hypotonic urinary tract angiography has the diagnostic value on the diagnosis of obstruction of urinary system disease," and are also helpful on the inflammatory lesions in the differential diagnosis, especially on the severe impairment of renal function of the upper urinary tract obstructive diseases. It can be used as the preferred method to examine the clinical urological patients with the advantages of less pain and good results.
