摘要
目的:探讨GE-64排螺旋CT低剂量扫描诊断输尿管阴性结石的临床应用价值。方法:回顾性分析2012年6月-2015年12月本院51例经静脉肾盂造影(IVP)诊断可疑为输尿管阴性结石患者的临床资料,经多层螺旋CT检查确诊,准确率达100%,同时分析X线腹部平片+静脉肾盂造影(KUB+IVP)与螺旋CT扫描两种检查方法诊断输尿管阴性的敏感性、准确率,并对两种方法检出结果做统计分析。结果:51患者中,KUB+IVP检查可疑阴性结石43例,阴性8例。螺旋CT扫描检查拟诊断输尿管结石51例,所有病例均经手术证实,其敏感性、准确率均为100%。两种检查方法的输尿管阴性结石的敏感性、准确率比较,差异均有统计学意义(P<0.05)。结论:螺旋CT扫描明显较KUB+IVP检查有较突出的优势,其具有较高分辨性,不受肠气及呼吸影响。结石CT值常大于300 HU,无论是含钙、磷、镁成分的结石即阳性结石,或是尿酸盐类结石即阴性结石在CT上均能清晰地观察到输尿管腔内较高密度的结石影像。螺旋CT对输尿管阴性结石具有重要的诊断价值。
Objective:To evaluate the clinical application value of the low-dose scanning of GE-64 spiral CT for the diagnosis of ureteral radioparent calculi.Method:The clinical data of 51 patients with ureteral calculi confirmed by venous pyelography(IVP)from June 2012 to December 2015 were retrospectively analyzed.The accuracy of 100%was checked by multilayer spiral CT,the sensitivity of KUB+IVP and the spiral CT scan were analyzed to diagnose the sensitivity and accuracy of the uretera negative,and the results of the two methods were analyzed.Result:51 KUB and IVP examination in patients with suspected negative stone 43 cases,8 cases of negative.Spiral CT scan in the diagnosis of 51 cases of ureteral calculi,all cases were confirmed by surgery,the sensitivity and accuracy were all 100%.The sensitivity and accuracy of the two methods were statistically significant(P<0.05).Conclusion:Spiral CT scan has obvious advantages over KUB+IVP,which has high resolution and is not affected by intestinal gas and respiration.Stone CT values often greater than 300 HU,calcium,phosphorus,magnesium,composition of positive stones,or uric acid salts or negative stones on CT can clearly observe the stone image ureteral lumen higher density.Spiral CT has important diagnostic value for ure teral negative calculus.
