活动期类风湿性关节炎患者IL-17和IL-27水平变化及其临床意义 被引量:3

摘要 目的分析活动期类风湿性关节炎(Rheumatoid arthritis,RA)患者血清和关节液中白细胞介素-17(Interleukin-17,IL-17)和白细胞介素-27(Interleukin-25,IL-25)水平,探讨IL-17和IL-27在活动期RA患者中的临床意义及其与相关实验指标的相关性。方法选择56例活动期RA患者为实验组,30例健康体检者为对照组,采用免疫散射比浊法测定血清C反应蛋白(CRP)和类风湿因子(RF)的浓度,用魏氏法测定红细胞沉降率(ESR),用化学发光微粒子免疫检测法检测抗环胍氨酸多肽抗体(CCP)浓度,用ELISA法测定健康体检者血清和RA患者血清及关节液中的IL-17、IL-27水平。结果 RA患者血清和关节液中IL-17和IL-27水平明显高于对照组;RA患者血清IL-17水平与ESR、CRP、CCP呈正相关性,关节液中IL-17水平与ESR、CRP呈正相关性,与RF无相关性;RA患者血清及关节液中IL-27水平与ESR、CRP、RF和CCP无明显相关性;RA患者血清及关节液中IL-17与IL-27水平呈正相关性。结论 IL-17和IL-27在RA患者的血清和关节液中过度表达,提示IL-17和IL-27参与了RA的免疫性致病机制,其具体作用尚待进一步研究。 Objective To determine the concentrations of IL-17 and IL-27 in serum and synovial fluid, and investigate their significance in the pathogenesis of activated rheumatoid arthritis(RA). Methods The serum concentrations of CRP and RF in 56 cases of activate RA patients and 30 cases of healthy controls were detected by immune scattering turbidimetry test. Erythrocyte sedimentation rate(ESR) was measured by Wei's method. The serum and synovial fluid concentrations of IL-17 and IL-27 were determined by ELISA. Results The concentrations of serum and synovial fluid IL-17 and IL-27 were significantly higher than those of healthy controls. Furthermore, there were positive correlations between RA serum IL-17 and ESR and CRP and CCP, and there were significantly positive correlations between RA synovial fluid IL-17 with ESR and CRP. There was a significantly positive correlation between RA synovial fluid IL-17 and IL-27, as well as RA serum. Conclusion IL-17 and IL-27 were over expressed in serum and synovial fluid of active RA patients, which revealed that Il-17 and IL-27 may play important roles in the inflammatory pathogenesis of RA. And the relationship between IL-17 and IL-27 need to be further studied.
