摘要
目的了解西安市男男性行为人群(MSM)艾滋病病毒(HIV)感染的流行趋势,为评价西安市艾滋病防控效果及制定防治策略提供依据。方法通过横断面调查的方法,对艾滋病哨点监测的MSM进行行为学调查及HIV、梅毒、丙型肝炎(丙肝)病毒的血清学检测。结果 2007-2013年,西安市MSM哨点检测的HIV抗体阳性率分别为3.13%(8/256)、3.25%(13/400)、3.75%(15/400)、8.00%(32/400)、5.50%(22/400)、8.25%(33/400)、9.17%(55/600),HIV感染率在上升(趋势χ2=23.09,P<0.01)。MSM最近一次肛交时安全套使用率(趋势χ2=38.18,P<0.01)及最近一年接受干预服务的比例(趋势χ2=130.63,P<0.01)显著上升,最近6个月商业同性性行为的比例显著下降(趋势χ2=40.79,P<0.01)。梅毒感染率没有显著变化(趋势χ2=0.11,P>0.05),丙肝感染率也维持在一个稳定的水平(趋势χ2=2.17,P>0.05)。结论西安市MSM的HIV感染情况相当严峻,梅毒、丙肝感染率维持在一个相对稳定的水平。目前采取的防治措施有效地改善了HIV在MSM中的传播,但仍需加强干预及检测力度,遏制艾滋病疫情向一般人群的蔓延。
Objective To analyze epidemic trend of human immunodeficiency virus （HIV） among men who have sex with men （MSM）, and provide evidence for evaluating effects and developing strategies of acquired immune deficiency syndrome （AIDS） control and prevention in Xi＇an. Methods Through a cross-sectional sentinel survey of MSM, behavior investigation and serological test were performed. Results From 2007 to 2013, the HIV positive rates of MSM in Xi＇an were 3.13% （8/256）, 3.25% （13/400）, 3.75% （15/400）, 8% （32/400）, 5.5% （22/400）, 8. 25 % （33/400） and 9.17 % （55/600）, respectively. The HIV prevalence increased significantly （P〈 0. 01）. The rates of condom use in the last anal sex（P〈0.01） and prevention service in the last year（P〈0.01） increased significantly, and the rate of commercial sex with men during the past 6 months declined significantly （P〈0.01）. The prevalences of Syphilis and HCV have not changed significantly （P〈0.05）. Conclusion The situation of HIV infection among MSM is quite grim in Xi＇an. The prevention measures have reduced HIV transmission of MSM effectively in Xi＇an, but intervention still needs to be strengthened to contain the spread of HIV to general population.
