摘要
目的探讨预见性护理在甲状腺手术后的应用效果及其影响因素。方法选择2012年6月—2013年6月在新疆医科大学第一附属医院血管甲状腺外科行甲状腺手术治疗的126例患者,随机分为对照组(52例)和试验组(74例),对照组患者住院期间进行一般护理及常规护理健康教育,试验组在对照组的基础上进行术前预见性护理。对两组患者的满意度、合作程度、焦虑状态、并发症及住院时间进行比较,并对干预效果和影响因素进行分析。结果试验组患者出院前的满意度、合作程度明显高于对照组,差异有统计学意义(P均<0.05);并发症发生率低于对照组,住院天数少于对照组,差异均有统计学意义(P<0.05);试验组SAI评分低于对照组,差异有统计学意义(P<0.05)。影响干预效果的独立因素有文化程度、民族、报销方式和婚姻状况(P均<0.05)。结论预见性护理在提高患者满意度、合作程度、缓解患者焦虑水平、降低并发症发生率和减少住院天数等方面有显著效果,值得在临床护理工作中推广应用。在实施过程中应注意加强对文化水平较低、独居、自费的及少数民族患者的干预强度。
Objective To investigate the predicative nursing intervention on thyroid operation and its influence factors. Methods From June 2012 to June 2013 in our department 126 case thyroid operation patients were randomly divided into 52 cases of the control group and experimental group of 74 cases, control group were imposed conventional nursing health education during hospitalization, the experimental group were applied predictive nursing on the basis of the control group. And then we compared the effect of two nursing mode in terms of patient＇s satisfaction, cooperation, anxiety, complications and hospitalization time. Finally we conducted multifactor analysis. Results Predictive nursing can significantly improve the satisfaction of patients before discharge, cooperation degree, and significantly reduce the incidence of complications occurrence rate, hospitalized days, and even ease the anxiety degree （P 〈0.05）. Influence factors affecting the effects of intervention include education, ethnic, reimbursement methods and marital status. Conclusion Predictive nursing care can significantly improve patient satisfaction, cooperation degree, ease the anxiety level of patients, reduce the incidence of complications. It is worthy to be popularized in clinical nursing work. In the implementation process we should pay attention to those patients with poor education, solitaries and minorities, and those without medical insurance.
