摘要
目的探讨氯吡格雷与阿托伐他汀对短暂性脑缺血(TIA)发作患者血液流变学的影响及临床疗效。方法以2012-01—2013-04我院神经内科收治的160例TIA患者为研究对象。对照组80例在对症、对因治疗的基础上给予氯吡格雷75mg/d,观察组80例患者在对照组的基础上联用阿托伐他汀20mg/d,连续治疗6个月。观察2组患者的血液流变学变化并评价治疗效果。结果治疗后,观察组患者的高切全血黏度、低切全血黏度、血浆黏度及纤维蛋白原的水平均明显低于对照组(P<0.05),近期疗效及远期疗效均明显优于对照组(P<0.05)。2组均有2例患者发生皮下出血,使用止血药后好转;观察组3例门冬氨酸转氨酶和(或)谷氨酸转氨酶轻度增高,给予护肝治疗,经复查不再升高。结论氯吡咯雷联用阿托伐他汀治疗TIA,可显著改善患者血液流变学,安全有效,值得临床推广。
Objective To explore the clinical efficacy of clopidogrel combined with atorvastatin in the treatment of transient ischemic attack （TIA） patients and influence on the hemorheology .Methods One hundred and sixty TIA patients in our hospital from January 2012 to April 2013 were studied .In control group ,80 cases of TIA patients were given clopidogrel （75 mg/d） based on symptomatic treatment .In observation group ,80 cases of TIA patients were given atorvastatin （20 mg/d） , clopidogrel （75 mg/d） based on symptomatic treatment .The hemorheology and the therapeutic effect were compared between two groups .Results After treatment ,the levels of high-shear whole blood viscosity ,low shear whole blood viscosity and plasma viscosity meter fibrinogen of observation group all significantly lower than those in control group （P〈0 .05） .Short-term effects and long-term efficacy were all significantly better than those in control group （P〈0 .05） .Two patients in observation group and two patients in control group showed subcutaneous bleeding ,and were improved by using hemostatic .There were three patients＇aspartate aminotransferase and/or alanine aminotransferase increased slightly in observation group .After the routine treatment ,these three patients were all back to normal .Conclusion The treatment of clopidogrel combined with atorvastatin can significantly improve TIA patients＇hemorheology .It is not only a safe but an effective treatment ,and is worth ap-plying to clinic ,and is widely developing .
