摘要
目的探讨胆管癌患者血清CA19-9的水平变化及临床意义。方法分析该院收治的37例胆管癌患者为观察组,同时随机抽取同期收治的胆管良性病变患者32例为对照组,采用化学发光微粒子检测法测定两组患者血清CA19-9水平。同时比较两组患者的阳性率。结果观察组血清及胆汁中CA19-9水平均显著高于对照组,两组比较差异有统计学意义(P<0.01,<0.005)。对照组与观察组阳性率分别为18.75%(6/32),72.97%(27/37),两组比较差异有统计学意义(P<0.005)。高分化腺癌CA19-9阳性率与低分化腺癌组阳性率比较差异有统计学意义(χ2=4.763,P<0.05);中、低分化腺癌CA19-9水平比较差异无统计学意义(χ2=2.250,P>0.05)。高、中、低分化腺癌组CA19-9水平相比较差异有统计学意义(F=4.125,P<0.05),高分化腺癌CA19-9水平与低分化腺癌组比较差异有统计学意义(t=2.174,P<0.05);中分化腺癌CA19-9水平与低分化腺癌组阳性率比较差异有统计学意义(χ2=2.077,P<0.05)。结论 CA19-9水平及阳性率检测在胆管癌的诊断中具有重要价值,有利于病情的评估。
Objective To investigate the dynamic changes and clinical significance of serum levels of CA19-9 in patients with bile duct carcinoma. Methods 37 cases of patients with bile duct carcinoma admitted in our hospital from January, 2011 to January, 2013 were selected as the observation group and analyzed. And 32 cases of patients with benign bile duct lesions admitted to the hospital during the same period were randomly selected as the control group. Chemiluminescent microparticle assay was used to measure the serum CA19-9 levels of two groups. The positive rate of two groups of patients was compared. Results The CA19-9 levels in serum and bile of the patients in the observation group were much higher than those of the patients in the control group, the differences between two groups were statistically significant（P〈0.01, 0.005）. The positive rate of the control group and the observation group was 18.75%（6/32）, 72.97%（27/37）, respectively, the difference between two groups was statistically significant（P〈0.005）. The difference in the positive rate of CA19-9 between well differentiated adenocarcinoma group and poorly differentiated adenocarcinoma group was statistically significant（χ2=4.763, P〈0.05）; but the difference in the positive rate of CA19-9 between moderately differentiated adenocarcinoma and poorly differentiated adenocarcinoma group was not statistically significant（χ2=2.250, P〈0.05）.
