乳腺癌组织CK5/6的表达及与预后关系的研究 被引量:9

Expression of CK5/6 in human breast cancer and its clinical significance for prognosis

摘要 目的:分析乳腺癌组织细胞角蛋白5/6(cytokeratin 5/6,CK5/6)表达及其与患者预后的关系。方法:收集北京协和医学院肿瘤医院2000-01-01-2000-12-31手术的乳腺癌石蜡组织标本共307例,制作成乳腺癌组织芯片。采用免疫组织化学MaxVision二步法检测乳腺癌组织CK5/6蛋白的表达,应用SPSS 15.0统计软件分析CK5/6与乳腺癌患者的年龄、月经、妊娠、哺乳、肿瘤大小、TNM分期、ER、PR和HER-2等因素之间的关系及在乳腺癌患者预后中的价值。结果:乳腺癌组织CK5/6阳性表达率为23.9%(68/285)。单因素生存分析显示,CK5/6蛋白的表达与患者的总生存率和无病生存率有关,P值分别为0.022和0.023;将分期、ER、PR、HER-2和CK5/6等指标进行Cox多因素生存分析,显示CK5/6是与总生存相关的独立的预后因素(P=0.023),乳腺癌组织CK5/6蛋白高表达提示有较差的临床预后。结论:CK5/6高表达的乳腺癌患者较低表达患者有较差的预后,CK5/6是与乳腺癌总生存相关的独立预后因素。 OBJECTIVE: To explore the expression of CK5/6 in breast cancer and the relationship with the prognosis of breast cancer. METHODS: Clinical and pathological parameters of 307 breast cancer patients who had been operated in the Cancer Hospital,Chinese Academy of Medical Sciences between 1st Jan. 2000 and 31st Dec. 2000 were collected. Tissue microarrays were established with wax blocked tissue. The expression of CK5/6 in 307 breast carcinomas samples was detected by MaxVision two-step immunohistochemistry. SPSS 15.0 was used to analyze the relationship between CK5/6 and the age, menses, pregnancy, lactation, tumor size, TNM staging, ER, PR, HER-2 and value in the prognosis of patients with breast cancer. RESULTS: Positive expression ratio of CK5/6 in breast cancer was 23.9 % (68/285). CK5/6 expression was not correlated with age, menses, size of tumor, clinical TNM stage (P〉0.05). Univariate survival analyses indicated that the expression of CK5/6 was associated with overall survival(P=0. 022) and disease free survival (P=0. 023). Multivariate analyses with ER, PR, HER-2,stage showed that the expression of CK5/6 was independent prognostic factor for overall survival(P=0. 023). The high expression of CK5/6 was correlated with the poor prognosis of breast cancer. CONCLUSIONS: Breast cancer with a positive expression of CK5/6 will result in poor prognosis and CK5/6 is the independent prognostic factor to evaluate the prognosis of breast cancer.
