摘要
对慢病管理现状进行了分析,对慢病管理运行机制及慢性病对人类健康的危害进行探析,指出应加强政府主导作用,调动社区服务机构及慢病管理人员主动性,引导老年人对慢性病的认识,有效利用有限资源达到最大的健康改善效果。
The investigation on chronic diseases management methods in community health services among the elderly in Xincheng District in Xi＇an was performed, the status quo of chronic diseases management was analyzed, and the operating mechanism of chronic diseases management and the harm of chronic diseases were discussed. The paper pointed out that to effective use of limited resources to achieve the greatest health improvement, several measures can be carried out including strengthening government leading role, mobilizing initiatives of community service organizations and chronic disease management staff, and guiding the awareness of chronic diseases among the elderly,
