摘要
目的：观察重比重与等比重布比卡因腰硬联合（CSEA）麻醉在剖宫产手术中的效果及对血流动力学的影响。方法：选择ASAⅠ～Ⅱ级,在腰硬联合麻醉下行剖宫产手术孕妇60例,随机分为重比重组和等比重组两组。分别注入0.5%重比重布比卡因和0.5%等比重布比卡因2 ml,注射速度为0.2 ml/s,观察各组麻醉起效时间、达峰时间、阻滞平面、麻醉效果、血压变化、不良反应和新生儿Apgar评分等指标。结果：两组均能取得较满意的麻醉效果,两组血氧饱和度、新生儿Apgar评分比较差异无统计学意义,但等比重组较重比重组达峰时间长,对患者血压影响较小,对运动神经阻滞程度较轻,不良反应少,差异均有统计学意义（P〈0.05）。结论：等比重布比卡因更适于剖宫产手术的麻醉。
Objective：To investigate the effectiveness and the influence to hemodynamic of heavy proportion and equal proportion bupivacaine in combined spinal-epidural anesthesia for caesarean section operation.Method：60 cesarean section operation patients with ASAⅠ-Ⅱwere randomly divided into heavy proportion group and equal proportion group with 30 cases each.2 ml 0.5%heavy proportion bupivacaine and equal proportion bupivacaine were injected to two groups at a speed of 0.2 ml/s.The onset time,peak time,sensory block level and the quality of analgesia were assessed.The changes of blood pressure,side effects,the Apgar score of newborn and other indexes were recorded.Result：The anesthetic effect of all cases was satisfactory.There were no significant difference in anesthetic effect,oxyhemoglobin saturation and Apgar score of newborn in two groups.Equal proportion group presented a longer peak time than heavy proportion group（P〈0.05）,but equal proportion had less effect on the blood pressure.Meanwhile,the movement recovery time of equal proportion was shorter.The side effect incidence rates in equal proportion group were lower than in heavy proportion group（P〈0.05）.Conclusion：Equal proportion bupivacaine is better in combined spinal-epidural anesthesia for caesarean section operation.
