摘要
目的对本中心无偿献血者2012年12月~2013年5月血液标本中HBsAg结果呈阳性的标本进行乙肝两对半、中和确证试验和HBV-DNA检测,了解无偿献血人群乙肝感染现状及血清学特性。方法无偿献血者血样采用1遍ELISA和1遍NAT检测的筛查模式,HBsAg筛查结果阳性的标本进行中和试验、抗-HBs、HBeAg、抗-HBe、抗一HBc检测,同时进行HBsAg、抗-HBs的定量检测。结果无偿献血人群中HBsAg酶免确证阳性率为0.63%(236/37669);血清学模式以HBsAg(+)、抗-HBe(+)、抗-HBc(+)为主,占73.31%;NAT有反应性和NAT无反应性的血清学模式构成比无统计学差异(P〉0.05);不同HBsAg浓度组间,NAT阳性检出率有统计学差异(P〈0.01),HB-sAg浓度〉100.00IU/mL,NAT阳性检出率达100%;在HBsAg和抗-HBs共存的模式中,NAT有反应性组和NAT无反应性组间无统计学差异(P〉0.05)。结论无偿献血人群中HBsAg筛查结果阳性以HBsAg(+)、抗-HBe(+)、抗-HBc(+)的血清学模式为主,HBsAg的含量和NAT检出率有相关性。
Objective Five makers of hepatitis B, neutralization test and HBV-DNA detection were carried out in the HBsAg screening positive blood samples from December 2012 to May 2013 to understand the hepatitis B infection status and serological characteristics in those blood donors. Methods Screening model was chose as 1 time ELISA and 1 time NAT. The samples with HBsAg positive results were detected by neutralization test, anti-HBs test, anti-HBeAg test, anti-HBe test,anti-HBc test,as well as a quantitative detection of HBsAg and anti-HBs at the same time. Results In the voluntary blood donos,the positive rate of HBsAg detected by ELISA was 0. 63% (236/37 669) ;mainly with serological patterns of HBsAg ( + ), anti-HBe ( + ), and anti-HBc ( + ) , accounting for 73.31%. There were no statistical differences (Х^2 = 8.40 ,P 〉 0. 05 ) in constituent ratio of serological pattern between NAT positive and negative group. A significant difference was found (X2 = 47. 58, P 〈 0. 01 ) in different HBsAg concentration groups in NAT positive samples. NAT positive rate was 100% when the HBsAg concentration was higher than 100 IU/mL. Between NAT positive and negative groups, no statistical differences (Х^2 = 1.39,P 〉 0. 05) were showed in the HBsAg and anti-HBs coexistence model. Conclusion HBsAg ( + ), anti-HBe ( + ), anti-HBe ( + ) were the mainly serological patterns of HBsAg screening positive results in blood donors,and the concentration of HBsAg showed some correlation with NAT positive rate.
