亚临床甲状腺功能减退与2型糖尿病病人微血管并发症的相关性研究 被引量:12

摘要 目的探讨2型糖尿病合并亚临床甲状腺功能减退症(亚临床甲减)病人亚临床甲减与糖尿病微血管并发症的相关性。方法选择山西医科大学第一医院内分泌科住院的2型糖尿病病人129例,根据甲状腺功能结果分为2型糖尿病合并亚临床甲减组(H组)和2型糖尿病甲状腺功能正常组(N组)。记录所有病人的年龄、性别、糖尿病病程、身高、体重和眼底检查结果,检测其血脂、糖化血红蛋白(HbA1c)、空腹血糖、空腹胰岛素、24h尿蛋白水平、甲状腺过氧化物酶抗体(TPOAb)和甲状腺球蛋白抗体(TGAb)。结果两组年龄、病程、体重指数(BMI)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、HbA1c、胰岛素抵抗指数(HOMA-IR)差异无统计学意义(P>0.05)。H组女性比例及TPOAb、TGAb水平均明显高于N组(P<0.05)。H组糖尿病肾病及糖尿病视网膜病变发生率均高于N组,其中糖尿病肾病的发生率两组间比较差异有统计学意义(P<0.05)。Logistic回归分析提示亚临床甲减是糖尿病肾病的独立危险因素(P<0.05)。结论亚临床甲减能够增加2型糖尿病病人糖尿病肾病变的发病风险,与糖尿病微血管并发症密切相关,2型糖尿病病人有必要筛查甲状腺功能。
