血清髓鞘碱性蛋白对颅脑损伤程度及预后价值的判断

摘要 目的探讨急性颅脑损伤患者检测血清髓鞘碱性蛋白(MBP)的浓度及动态变化,对颅脑损伤的诊断分型、评估伤情和判断预后的临床价值。方法采用放射免法分析,对185例急性颅脑损伤患者病例组及50名健康体检人群对照组血清MBP浓度进行检测,于伤后24 h,3 d、7 d、10 d MBP的浓度进行动态检测和观察。结果患者组伤后24 h血清MBP浓度显著增高,与对照组差异有统计学意义(P<0.001),与GCS评分呈负相关关系(r=-0.6661,P<0.05),动态检测血清MBP浓度显示重度颅脑损伤患者虽经积极救治,血清MBP浓度持续升高,提示预后不佳,反之则预后相对较好。结论检测颅脑损伤后患者血清MBP浓度可以直接反映脑细胞损害程度,是一项较为敏感的生化指标,特别是动态检测血清MBP浓度对判断颅脑损伤后伤情评估、疗效观察及预后具有一定的临床意义。 Objective To explore the detection of serum myelin basic protein ( MBP) concentration and dynamic changes in brain injury so as to provide clinical basis for diagnostic classification ,trauma evaluation and prognosis prediction .Methods 185 cases of acute traumatic brain injury patients and 50 cases of healthy controls&#39;serum MBP concentrations were detected and analyzed,24 h,3 days,7 days and 10 days after injury,using radioimmunoassay .The concentration of MBP was dynamical-ly observed .Results 24 hours after injury ,serum MBP of the acute brain injury group considerably increased compared with the normal control group.The difference was statistically significant (P &lt;0.001),and was negatively correlated with GCS scores(γ=-0.6661,P&lt;0.05).Dynamic detection of serum MBP concentration indicated that the increased MBP concentra-tion after active treatment might forecast poor prognosis , and vice versa , relatively good prognosis .Conclusion Detection of serum MBP concentration in brain injury patients can directly reflect the extent of brain damage .It may be regarded as one of more sensitive biochemical indicators .Dynamic detection of serum MBP concentration is of clinical significance for injury as -sessment,therapeutic observation and prognosis judgment .
